Whereas the Diagnostic-Cohort spanned the full biological spectrum of amyloid deposition, the Eligibility-Cohort displayed a more dichotomous and advanced biological pattern despite earlier clinical staging. Patients referred for treatment were either unexpectedly PET-negative (despite CSF-results) or showed high CL values even at the MCI stage. These findings suggest that the more "typical" AD clinical presentation among patients currently selected for DMTs in real-world settings may be associated with more advanced biological staging despite mild clinical symptoms, highlighting a shift in patient profiles as the workflow moves from diagnosis to eligibility."
