IN VIVO MONITORING OF CAR-T CELL THERAPY: THE ROLE OF MULTIPARAMETRIC FLOW CYTOMETRY IN LYMPHOCYTE EXPANSION ANALYSIS (SIE 2026) - Oct 6, 2026 - Abstract #SP138; "The study focus- es on quantifying expansion phases and identifying specific CAR-T cellular phenotypes that correlate with therapeutic ef- ficacy and potential toxicities (such as CRS and ICANS).This research analyzed 18 patients with different hematological malignancies undergoing CAR-T therapy with five different commercial products (Tisa-cel, Ide-cel, Brexu-cel, Axi-cel, and Liso-cel), focusing on real-time expansion monitoring through flow cytometry. The results confirm a three-phase CAR-T expansion kinetic profile with peak on the eighth day, mainly determined by CD8+ effector and central memo- ry subsets, suggesting a crucial role in long-term CAR-T per- sistence and therapeutic durability. While early expansion may contribute to ICANS development, long-term CAR-T per- sistence does not appear to influence its onset. Regarding clinical efficacy, outcomes are more closely linked to sus- tained expansion and cellular persistence rather than the ini- tial..."
