Most will receive a second IMiD (e.g., pomalidomide; Pom) during their course...We performed whole genome sequencing (WGS) for patients on the ReKInDLE study of Iberdomide, Carfilzomib, Daratumumab, and dex (Iber-DKd; n=32) for Len-exposed early-relapsed MM, with de-escalation to Iber monotherapy after 8 cycles... CELMoDs have activity in IMiD-resistant settings but discrete resistance variants apart from CRBN mutations appear to influence response, arguing for screening in IMiD-resistant scenarios, and for earlier CELMoD integration prior to selection of universal IMiD/CELMoD resistance.
