Immunomodulatory Salvage Reverses Primary Refractoriness to Bispecific T-Cell Engagers in Multiple Myeloma (IMS 2026) - Sep 11, 2026 - Abstract #PA-239; Pres time: Sep 24, 2026; " We report five patients (pts) with RRMM treated between 2023 and 2026 in three tertiary cancer centers who were primarily refractory or have shown progressive disease after at least one full cycle of teclistamab (n=2) or talquetamab (n=3). All 5 pts received immunomodulatory salvage with pomalidomide (POM) added to ongoing TCE therapy (POM mono, n=2) or initiated shortly after TCE discontinuation (daratumumab(DARA)-POM-cyclophosphamide(CY)-dexamethasone(dex), n=1; POM-CY-dex, n=1; elotuzumab-POM-dex, n=1)... Response depth and kinetics exceeded what is expected from used salvage regimens alone. Persistent TCE concentrations after discontinuation together with POM-based salvage led to a de facto combination therapy. This combination was able to reverse functional TCE resistance, even in pts refractory to the salvage drugs."
