Fibroblast-rich immune-excluded tumors with activated TGF-β signaling derive preferential benefit from bintrafusp alfa plus paclitaxel. These findings support a model in which TGF-β blockade overcomes immune exclusion and identify a biologically distinct responder population that may not be captured by conventional biomarkers for PD-1/ PD-L1–based therapy."
