PRE-EMPTIVE CORTICOSTEROIDS FOR TOXICITY MITIGATION IN CD28- COSTIMULATED CD19 CAR-T CELL THERAPY: AN ITALIAN SINGLE-CENTER EXPERIENCE (SIE 2026) - Oct 6, 2026 - Abstract #DP051; Pres time: Sep 21, 2026; 07:00 PM - 08:00 PM; Location: AREA POSTER; "This retrospective, sin- gle-center study evaluates the impact of pre-emptive corti- costeroid therapy in a cohort of patients receiving CD28-cos- timulated CAR T-cells (axi-cel and brexu-cel). According to Institutional Procedures from 2022, methylprednisolone (2 mg/kg/day or dexamethasone equivalent) has been adminis- tered to all patients with persistent (>24h) grade 1 CRS, re- fractory to 1–3 doses of tocilizumab; tapering was started on the first day following CRS resolution...These findings show that the inci- dence of grade ≥3 CRS, ICANS, and grade ≥3 ICANS in a cohort of patients treated with pre-emptive steroid therapy in case of persistent grade 1 CRS is markedly lowercom- pared to the rates reported in pivotal studies such as ZU- MA-1 and ZUMA-4 trials (grade ≥3 CRS: 13-15%; ICANS: 64-63%; grade ≥3 ICANS: 28-31%) as well as in multiple re- al-world studies. Notably, the OS and PFS observed in this cohort were consistent with current literature,..."
