OUTCOMES IN TCRΑΒ-DEPLETED HAPLOIDENTICAL TRANSPLANTATION WITH UPFRONT CD45RA-DEPLETED MEMORY T-CELL ADD-BACK: ANALYSIS OF 127 ADULTS WITH HEMATOLOGICAL MALIGNANCIES TREATED AT NATIONAL UNIVERSITY CANCER INSTITUTE, SINGAPORE (EBMT 2026) - Mar 14, 2026 - Abstract #B017; Pres time: Mar 24, 2026; 06:00 PM - 07:00 PM; Location: VELAZQUEZ; "Most patients (n=119) received reduced-intensity conditioning comprising fludarabine, thiotepa, and melphalan combined with either total lymphoid irradiation (TLI 6-7.5 Gy; n=22), low-dose total body irradiation (TBI 2 Gy; n=105), or anti-thymocyte globulin (ATG; n=1)...GVHD prophylaxis (tacrolimus n=117; sirolimus n=2; MMF n=4) was administered for 30 days post-transplant...Outcomes significantly improved in transplants performed after 2020, coinciding with universal adoption of TBI 2Gy and letermovir prophylaxis. RIC haplo-HCT using TCRαβ-depleted grafts with upfront CD45RA-depleted memory T-cell achieves reliable engraftment, remarkably low rates of severe acute and chronic GVHD, translating into promising GRFS. Optimal outcomes occur in patients with low/intermediate-risk DRI receiving TBI 2Gy conditioning." Clinical • Acute Graft versus Host Disease • Chronic Graft versus Host Disease • Graft versus Host Disease • Hematological Malignancies • Immunology • Infectious Disease • Novel Coronavirus Disease • Oncology • Septic Shock • Transplantation • CD34
