In this pilot real-world cohort of CMV R+ kidney transplant recipients, letermovir prophylaxis initiated early after transplantation and continued for approximately 200 days was safe and effective. No CMV DNAemia or acute rejection episodes occurred during prophylaxis, and hematologic toxicity was absent.
