De Novo Letermovir for Cytomegalovirus Prophylaxis in High Risk Liver Transplant Recipients (WTC 2025) - Jul 30, 2025 - Abstract #P2.05.18; Pres time: Aug 4, 2025; 03:30 PM - 04:30 PM; Location: Exhibit Hall AB; "While valganciclovir (VGC) is the guideline-endorsed standard-of-care (SOC) agent for CMV prevention after LT, letermovir (LTV) represents a promising option given its reduced incidence of myelosuppressive toxicity.* Adult patients receiving a D+/R- LT between 6/1/2021 and 6/6/2024 were evaluated. De novo LTV for primary CMV prophylaxis after high-risk LT appears to be safe and effective. Our data suggests LTV is more likely to be successfully completed than the current SOC and is associated with less myelosuppressive toxicity which allows maintenance of higher mycophenolate doses during the first year. Future studies are needed to evaluate the impact of LTV on rejection rates and transplant outcomes."
