We present a unique case of severe disseminated HSV infection in a LTx recipient. Due to the known high mortality, consideration for HSV prophylaxis along with regularly reviewing safe sex practices is imperative in transplant recipients. While disseminated HSV is rare in solid organ transplant recipients, the diagnosis should remain on the differential in patients presenting similarly."
