Pacritinib-Based Salvage Therapy in High-Risk JAK2 V617F–Positive Polycythemia Vera With Recurrent Arterial and Venous Thrombosis: A Case Report (SOHO 2026) - Sep 1, 2026 - Abstract #MPN-348; "Despite antiplatelet therapy, anticoagulation, hydroxyurea, and subsequent ruxolitinib, the patient developed further thrombotic complications, including recurrent cerebrovascular events, bilateral upper- and lower-limb ischemia, and left ventricular thrombosis. This case describes a highly thrombotic, treatment-challenging PV phenotype in which pacritinib-based salvage therapy was associated with improved hematological control, cessation of venesection, and absence of further thrombosis during follow-up. Although the pacritinib was off-label use and this case study is hypothesis-generating, the findings from this case study support further evaluation of pacritinib in selected patients who have refractory PV."
