Epigenetic mutations represent markers of clonal evolution and are associated with a more aggressive disease phenotype in MPNs. Their co-occurrence with spliceosome mutations indicates advanced clonal complexity for PMF. These alterations identify a high-risk subgroup of patients and may guide therapeutic stratification, including selection of patients who may benefit from Janus kinase inhibitor therapy."
