Although urinary tract malignancies have been described in IBD patients receiving thiopurines, metastatic UTUC is rarely described and may initially mimic persistent infection, delaying diagnosis. This case highlights the importance of maintaining suspicion for genitourinary malignancy in IBD patients with longstanding thiopurine exposure and persistent urinary symptoms despite antimicrobial therapy."
