Comparative Real-World Safety of Linaclotide, Plecanatide, and Lubiprostone: Risks of AKI and Hemodynamic Events (ACG 2026) - Aug 29, 2026 - Abstract #P3077; Pres time: Oct 12, 2026; 10:30 AM - 04:15 PM; Location: Exhibit Halls A - D (Level 3); "After matching, 40,131 linaclotide-plecanatide and 115,091 linaclotide-lubiprostone pairs were identified. Compared with plecanatide, linaclotide users had higher 30-day risks of AKI (0.47% vs 0.24%; RR 1.95, 95% CI 1.53-2.49, p< 0.0001), volume depletion (0.53% vs 0.40%; RR 1.32, 95% CI 1.07-1.61, p=0.008), hypotension (0.70% vs 0.59%; RR 1.20, 95% CI 1.01-1.42, p=0.04), and diarrhea (1.32% vs 1.04%; RR 1.27, 95% CI 1.11-1.44, p=0.0003). When compared with lubiprostone, linaclotide users experienced lower risks of AKI (0.59% vs 0.74%; RR 0.80, 95% CI 0.73-0.89, p< 0.0001), volume depletion (0.61% vs 0.76%; RR 0.81, 95% CI 0.73-0.89, p< 0.0001), and hypotension (0.83% vs 0.95%; RR 0.88, 95% CI 0.80-0.95, p=0.002), with similar rates of diarrhea (1.30% vs 1.30%; RR 1.00, 95% CI 0.93-1.07, p=0.94)."
