In this multicenter Korean R/R CLL cohort, 6-month WES captured early treatment-associated clonal suppression but did not reliably distinguish late progressors from sustained responders, who were genomically indistinguishable at both baseline and 6 months despite divergent outcomes. Classical resistance, including PLCG2 mutation, emerged only on extended sequencing, whereas a genomically quiet late profile accompanied durable response. These findings indicate that a single early timepoint is insufficient and support longer-term serial genomic monitoring during continuous BTK inhibitor therapy."
