In this preliminary single-center experience, low-dose daratumumab was associated with marked DSA reduction and improved histologic activity at 6-month follow-up, with a favorable short-term safety profile. A high baseline DSA burden may identify patients less likely to achieve complete response with limited dosing. These findings support prospective studies to refine dosing strategies and evaluate durability of response and long-term allograft outcomes."
