ABOi kidney transplantation can be successfully performed despite extreme baseline isoagglutinin titers and perioperative titer rebound. A rigorous, multi-modal desensitization strategy combining IA, DFPP, and aggressive induction, paired with strict postoperative pharmacological and histological surveillance, can prevent hyperacute rejection and achieve excellent early allograft outcomes. High baseline and transplant-day anti-ABO titers do not always preclude transplantation."
