Area under the curve-guided mycophenolate mofetil dose optimization in an ABO-incompatible Thai kidney transplant recipient with rare thrombocytopenia: A case report (TTS 2026) - Sep 17, 2026 - Abstract #P2.355; Pres time: Sep 22, 2026; 04:45 PM - 06:00 PM; Location: Hall 1-2; P2; "Her regimen included a single dose of rituximab, followed by tacrolimus, MMF (1.5 g/day), and prednisolone; cytomegalovirus prophylaxis with valganciclovir was also initiated 28 days prior to surgery due to the high risk of rejection...It was only after a further reduction to 750 mg/day that the AUC decreased into the therapeutic range (33 mg·h/L), allowing for the resumption of ganciclovir... Although routine TDM for MMF is not universally recommended, this case underscores its clinical benefit when managing thrombocytopenia through MMF AUC-guided dose adjustment in high-immunological-risk cases, such as ABO-incompatible transplants. This study indicates that MMF AUC-guided TDM is an essential tool for precise dose optimization, balancing the prevention of rejection with the mitigation of adverse drug events."
