Porto-Sinusoidal Vascular Disorder (PSVD) Associated with Tacrolimus in a Kidney Transplant Recipient (TTS 2026) - Sep 17, 2026 - Abstract #P2.336; Pres time: Sep 22, 2026; 04:45 PM - 06:00 PM; Location: Hall 1-2; P2; "Two years post-transplant, patient was diagnosed with chronic antibody-mediated rejection (cAMR) and treated with plasmapheresis, IVIg, and rituximab; during this period, tacrolimus levels were maintained between 7–10 ng/mL...Given the absence of other known triggers, extended-release tacrolimus was identified as the potential offending agent and was switched to cyclosporin...Clinicians should consider PSVD as a differential diagnosis for unexplained portal hypertension in transplant recipients, even when traditional triggers like azathioprine are absent. The rapid clinical recovery following the cessation of tacrolimus strongly supports a causal link."
