Beyond Seronegative Ambiguity: 18f-FDGPET Illuminates What MRI Cannot in Pediatric Autoimmune Encephalitis — Insights from a Single Centre Study (EANM 2026) - Sep 29, 2026 - Abstract #EPS-243; Pres time: Oct 21, 2026; 08:00 AM - 09:30 AM; Location: Hall G2; "Critically, in all 7 MRI-normal patients, PET/CT provided the sole objective evidence of cortical dysfunction, directly informing escalation of immunotherapy (corticosteroids, IVIG, or rituximab) in 6 of these cases... ¹⁸F-FDG-PET/CT demonstrated abnormal cerebral metabolic patterns in all paediatric patients with suspected AIE, including those with normal MRI and seronegative antibody profiles. The characteristic pattern of multifocal hypometabolism with relative basal ganglia hypermetabolism supports AIE diagnosis and guides immunotherapy decisions. FDG-PET/CT should be considered an integral functional neuroimaging tool in paediatric AIE workup, particularly in resource-limited settings where comprehensive antibody panels are unavailable."
