(EANM 2026) - Sep 29, 2026 - Abstract #EP-0911; "Aim/Introduction: To evaluate the safety profile and therapeutic efficacy of dose-escalated ¹⁷⁷Lu PSMA-617 therapy in patients with metastatic castrate-resistant prostate cancer (mCRPC) post-taxane therapy, and to establish optimal dosing strategies by investigating whether higher activity doses (>200 mCi) can be safely administered without exceeding dose thresholds to critical organs including salivary glands, kidneys, and bone marrow... Preliminary experience indicates that dose-escalated ¹⁷⁷Lu PSMA therapy beyond conventional 7.4 GBq dosing is feasible and safe with improved objective response rates at higher activities (280-300 mCi/dose). Critical organ doses remain well below established thresholds. These findings support further investigation of optimized, individualized dosing strategies to maximize therapeutic benefit while maintaining acceptable toxicity profiles in mCRPC patients."
