Early metabolic response (Δ MTV) is the dominant driver of PFS, outperforming baseline MTV and clinical variables. Serial 18F FDG PET-CT surveillance allows for robust risk stratification; specifically, patients with a >90% reduction in MTV at 3 months may transition to less frequent surveillance, potentially reducing the total number of required PET scans by nearly half. Conversely, failure to achieve a complete metabolic response (defined as MTV=0 even in the presence of visually assessable residual disease) by the third PET scan indicates a high risk of treatment failure."
