TPO RECEPTOR AGONISTS VERSUS PLATELET TRANSFUSION FOR PROCEDURAL THROMBOCYTOPENIA IN CIRRHOSIS: A PROPENSITY-MATCHED COHORT STUDY (AASLD 2026) - Oct 6, 2026 - Abstract #1691; Pres time: Nov 5, 2026; 12:00 PM - 01:00 PM; Location: Convention Center: Poster Hall, Hall DE; "Platelet transfusion has historically been the standard peri-procedural strategy, but TPO receptor agonists (TPO-RAs) — including romiplostim, eltrombopag, avatrombopag, and lusutrombopag — offer an oral alternative for procedural platelet optimization. In this propensity-matched cohort of cirrhotic patients with thrombocytopenia undergoing invasive hepatic procedures, TPO-RA use was associated with consistently lower point estimates across GI bleeding, blood transfusion requirement, HE, and allcause mortality compared to platelet transfusion, including a near-significant reduction in transfusion burden (p = 0.055) and a 165-day median survival advantage. While no outcome reached statistical significance, likely reflecting the modest cohort size post-matching, the directional consistency of these findings supports the hypothesis that procedural platelet optimization with TPO-RAs may confer clinical benefit beyond simple platelet count elevation.
