Routine bedside count-rate monitoring during [177Lu]RLT is feasible and may enable structured early triage. Early identification of marked injection-site asymmetry may be relevant to optimise local clearance and dosimetric assessment. A two-level composite operational strategy combining bedside vigilance and a stricter urgent-imaging trigger may provide a pragmatic balance between early detection and avoidance of unnecessary scintigraphic escalation."
