In glioma, PSMA-targeted probes predominantly bind to non-neoplastic cells, creating a fundamental dosimetric barrier that explains therapeutic failure despite high PET uptake. These findings necessitate integrating cellular target validation and preemptive dosimetry into theranostic agent development."
