Retreatment with 177Lu-PSMA-617, successfully induced a second significant remission, supporting the feasibility of sequential PSMA-targeted radioligand therapy. However, rapid relapse four months after the fourth retreatment cycle, followed by complete PSA non-response to two reserved cycles, underscores a critical limitation of planned cycle reservation: disease biology can shift rapidly, rendering deferred therapy ineffective."
