This case illustrates how a single PET radiotracer can overlook clinically significant disease in metastatic medullary thyroid cancer (MTC). By integrating SSTR, 18F-DOPA, 18F-FDG, and 68Ga-FAPI PET/CT, we uncovered significant lesion-level heterogeneity and identified FAPI-targeted therapy as the most pertinent next theranostic option. For selected, heavily pretreated MTC patients, a customized multitracer approach may offer a more comprehensive understanding of disease biology and facilitate more personalized treatment decisions."
