[¹⁷⁷Lu] Lu-PSMA RLT is associated with significant nephrotoxicity systematically underdetected by eGFR. Higher baseline mGFR predicting greater nephrotoxicity risk likely reflects floor effects in already compromised kidneys and greater functional proximal tubular mass available for PSMA mediated radiation exposure with direct implications for risk stratification. These prospective data from an Indian cohort, the first to employ mGFR in this setting, support integration of measured GFR at key decision points during RLT."
