Quantitative SPECT/CT-based dosimetry in [177Lu]LuPSMA therapy remains dependent on the imaging system despite phantom calibration. These findings indicate that phantom-based calibration alone is insufficient to ensure clinical comparability of dosimetric estimates. This has direct implications for multicenter studies and for the transferability of dose-toxicity relationships, highlighting the need for harmonization or correction strategies before applying universal dosimetric thresholds in clinical practice."
