Two-Year Outcomes of Microwave Ablation for Recurrent Monoarthritis with Synovial Hypertrophy: Expanded Cohort and Long-Term Follow-Up Results

- "After surgical asepsis and local anesthesia, antimicrobial prophylaxis with ampicillin/sulbactam and sedation with midazolam and fentanyl were applied. MWA proved to be a safe and effective minimally invasive technique for managing RM with synovial hypertrophy, showing no complications. Over two years, the outcomes consistently aligned with initial findings, demonstrating significant reductions in the frequency of RM attacks and need for IAA, with additional substantial improvements in functional disability and pain scores. While persistent synovial fluid and hypertrophy were occasionally observed in the early months, significant regression of synovial hypertrophy was evident by the sixth month on MRI, correlating with sustained clinical improvements."

Typhi...Our study highlights the possibility of rapid dissemination of antibiotic resistance due to chromosomal point mutations and plasmid-mediated gene transfer. An Unexpected Presentation of MRSA Pneumonia

- "Empiric treatment with piperacillin-tazobactam was initiated...Pleural fluid cultures grew Streptococcus anginosus and antibiotic coverage was narrowed to ampicillin-sulbactam...Antibiotics were broadened to cefepime, vancomycin, and metronidazole...Patient was continued on all three antibiotics and therapeutic IV heparin was initiated. Once the patient showed clinical improvement on day 9, antibiotic coverage was narrowed to IV linezolid and later to oral doxycycline to complete a total antibiotic course of 36 days...However, despite a high negative predictive value (96.1%-99.8%), nasal MRSA screens may fail to detect pulmonary MRSA infections, so it remains a possibility that MRSA was a contributing pathogen on admission. Thus, providers should maintain a high degree of suspicion when there is evidence of clinical deterioration."

- "Oral amoxicillin-clavulanic acid was the most commonly used antibiotic before admission (28%) and intravenous ampicillin-sulbactam (45%) was the most frequently administered during hospitalization.Median hospital stay (3.8 vs. 2.3 days), imaging costs (16.9 USD vs. 11.9 USD), and total hospital costs (96.4 USD vs. 42.8 USD) were significantly higher in the Pre-AB+ group (p = 0.005, p = 0.016, p = 0.044, respectively). Conclusions/Learning Points Prior antibiotic use in children with cervical lymphadenopathy was associated with longer hospital stays, higher imaging expenses, and increased total costs. Rational antibiotic use and close outpatient monitoring are crucial for improving outcomes and reducing healthcare costs."
