Comparison of opioid-free versus weak-opioid general anesthesia on quality of postoperative recovery in soldiers undergoing arthroscopic meniscal surgery.

- "Anesthesia induction consisted of alfentanil 0.2 μg/kg in the WOA group and esketamine 0.2 mg/kg in the OFA group...The WOA group received remifentanil 0.1 μg/kg/h and sevoflurane (MAC 0.8-1.0)...Postoperatively, the OFA group reported lower NPRS scores across the first 3 days, required less rescue analgesia, and had a lower incidence of rebound pain. Compared with weak-opioid anesthesia, opioid-free anesthesia significantly improves early postoperative recovery quality, as measured by the QoR15, in soldiers undergoing arthroscopic meniscal surgery."

Our finding of limited opioid research in pediatric oncology in LMICs highlights a significant research gap and possibly limited access to opioids for pain in LMICs in pediatric oncology. In the context in which opioid distribution is unequal and inequitable generally, this is especially concerning for the vulnerable pediatric oncology population."

- "Midazolam use remained unchanged (mean 2.6 mg), but increased alfentanil dosing (518.4 µg vs. 862.9 µg) notably reduced significant discomfort (29% vs. 2.5%). Repeat EBUS-TBNA adds diagnostic value when used for IHC and molecular testing, but its use is limited following initial negative result. Sedation optimization and sampling of all abnormal stations should be prioritized. Alternative diagnostics should be considered beyond 2 attempts."

- "XXXXX recently added brain to its fentanyl analog testing method for 14 analogs (fluoroisobutyryl fentanyl, acetyl fentanyl, acryl fentanyl, alfentanil, butyryl fentanyl, carfentanil, fentanyl, para-fluorofentanyl, furanyl fentanyl, methoxyacetyl fentanyl, norcarfentanil, norfentanyl, sufentanil, and valeryl fentanyl) and 3 U-series drugs (U-47700, U-48800, and U-49900). Brain homogenate stability was improved when frozen and was not impacted by the addition of 1% sodium fluoride. The study herein provides insight into the feasibility of testing brain for fentanyl analogs and their stability under various storage conditions, contributing valuable data to the limited literature on brain toxicology testing."
