Beyond the Usual Suspects: Cracking the Case of AA Amyloidosis in the Gut
(ACG 2025)
- "We present a case of chronic diarrhea secondary to gastric amyloidosis.Case Description/ A 76-year-old female with a history of Waldenström macroglobulinemia (MYD88 mutation), renal amyloidosis, prior pulmonary embolism (on rivaroxaban), and stage IV marginal zone lymphoma (treated with R-CHOP in 2013), was transferred from a care facility due to hypotension. She reported a 10-month history of chronic diarrhea, characterized by 2–3 episodes daily, which began after a course of amoxicillin...A3, B3, C3, D3. When Congo Red stain is polarized, the amorphous material demonstrates apple green birefringence consistent with Amyloidosis in stomach, duodenum, jejunum, and colon, respectively."

Pharmaceutical quantification by UPLC-MS
(ACS-Fall 2025)
- "Amoxicillin, a widely used antibiotic, and leucovorin, an antidote to the harmful effects of chemotherapy, are frequently found to be substandard, leading to harmful consequences such as increased antimicrobial resistance and poor patient health outcomes. By optimizing injection volumes, generating reliable calibration curves, and evaluating matrix effects, our methods enable reproducible active pharmaceutical ingredient (API) detection at nanomolar concentrations. By leveraging UPLC-MS, DPAL enhances its capacity to detect a broader range of pharmaceutical quality issues, ultimately supporting global efforts to improve medicinal safety and public health."

Ten years of the distributed pharmaceutical analysis laboratory (DPAL)
(ACS-Fall 2025)
- "DPAL partners in LMICs provide samples of pharmaceutical products such as amoxicillin capsules, ciprofloxacin tablets, and dosage units of MESNA or leucovorin calcium, and analytical chemistry students perform system suitability testing, evaluate the quality of the pharmaceutical dosage forms, and may apply other analytical techniques to learn more about products that fail assay. Sample metadata, provenance, and test results are shared between the LMIC collection sites and the DPAL laboratories through a secure database called VERIFY. This presentation will highlight how the program has evolved over a decade."

Prevalence of self-medication with antibiotics among nonmedical students at Kabul University, Afghanistan: A cross-sectional study.
(PubMed, Medicine (Baltimore))
- "The most commonly self-medicated antibiotics were Amoxicillin 277 (75.1%), Azithromycin 138 (37.4%), and Penicillin 108 (29.3%), which were most commonly used for sore throat 190 (51.5%), common cold 181 (49.1%), and runny nose 166 (45%). Most advice on antibiotics comes from informal sources, such as friends and family, and the internet is also influential. Emphasis should now be placed on strengthening health education and improving access to healthcare services to mitigate the risks associated with the improper use of antibiotics."

Detection and antimicrobial susceptibility of E. coli O157:H7 in the milk supply chain, milking environments and humans in Woliata Sodo, Ethiopia.
(PubMed, BMC Microbiol)
- "This study confirmed the presence of multidrug resistant E. coli O157:H7 in the tested sample. Notably, the highest prevalence was detected in milk samples from collection centers, underscoring post harvest contamination as a critical point of intervention. Identification of the pathogen on milkers' hands, farm floors, and human stool samples highlights potential transmission pathways and suggests a tangible risk of foodborne infections. These findings emphasize the critical need for targeted public health strategies, including comprehensive education on hygienic practices and the implementation of effective contamination control measures, to reduce the risk of infection and protect consumer health."

Effects of antibiotic prophylaxis during labour on maternal and neonatal outcomes in women planning vaginal birth.
(PubMed, Cochrane Database Syst Rev)
- "Offering a single oral dose of prophylactic antibiotics to pregnant women in labour after 28 gestational weeks or more probably reduces maternal sepsis. It likely results in little to no difference in maternal mortality, and results in little to no difference in neonatal sepsis, neonatal mortality, perineal wound infection, and NICU admission. Evidence on antimicrobial resistance is very uncertain: short-term increases in resistant organisms were observed in some maternal samples, but no persisting between-group differences were found at 11 to 13 months' follow-up. Implementation should be cautious and accompanied by antimicrobial stewardship and AMR surveillance, and future research should better quantify AMR effects and identify optimal antibiotic strategies."

Accelerated bioremediation of antibiotics and nutrients in complex matrices using self-acclimatized, cost-effective microalgae‑bacteria consortia: a comprehensive comparison with standalone systems.
(PubMed, Environ Sci Pollut Res Int)
- "This study explored the simultaneous removal of major antibiotics (amoxicillin (AMX), ciprofloxacin (CIP), and sulfamethoxazole (SMX)) and nutrients using microalgae, bacteria, and their consortia. The study highlights the potential of consortia (dominated by Proteobacteria, Cyanobacteria, and Tetradesmus) for efficient removal of both antibiotics and nutrients in complex matrices. Overall, consortia reduce the need for aeration and illumination while producing beneficial by-products like EPS and lipids, contributing to more sustainable and cleaner water treatment solutions."

H. pylori Therapy: Susceptibility-Based Proton Pump Inhibitor Triple Therapy Is Less Expensive and More Effective Than Empiric Vonoprazan Triple Therapy
(ACG 2025)
- "The costs for generic esomeprazole-clarithromycin triple therapy is ~$50...When vonoprazan-amoxicillin dual therapy is optimized to reliably produce cure rates >90% it would be an effective but very expensive empiric treatment option.1... The price of empiric vonoprazan-clarithromycin triple therapy is approximately twice that of equally or more effective, susceptibility-based tailored PPI-containing triple therapy (e.g., $1,000 vs. $550). NGS susceptibility testing provides data regarding susceptibility to all available antibiotics (eg, provides more treatment options)."

Sarcina Esophagitis Complicating a Large Hiatal Hernia
(ACG 2025)
- "The patient was treated with oral amoxicillin and metronidazole for 7 days with eventual resolution of symptoms...Treatment with antibiotics is effective, and important to avoid disease complications. Most literature has focused on impaired gastric dysmotility as a risk factor for Sarcina related disease, but other conditions that promote food stasis in the esophagus, such as hiatal hernias, may play a role in the pathogenesis of Sarcina related disease."

Assessing Helicobacter pylori Testing and Treatment by Providers at an Academic Tertiary Medical Center: A Quality Improvement Project
(ACG 2025)
- "To assess baseline knowledge, in 2018 we performed an IRB approved survey of primary care and GI providers and found 20% were considering serology as a diagnostic option, 50% were treating asymptomatic patients with HPI, 15% were primarily treating with PPI-Bismuth-Metronidazole-Tetracycline (PBMT), 38% were always checking for eradication and 98% said local resistance information for HPI would be helpful. 108 providers participated in the re-assessment survey (Table 1). Key results include that 8% of providers are using serology for diagnosis, 69% are treating asymptomatic patients with HPI, 81% primarily prescribe PBMT and 78% always tested for eradication (Table 2). 14 % of providers are primarily prescribing PPI-amoxicillin-clarithromycin (PAC). Compared to 2018, 5 times more providers are primarily prescribing PBMT and almost 2 times more are consistently testing for eradication, likely as a result of updated national guidelines and local educational and..."
