Revisiting acute chest syndrome (ACS) associated with sickle cell disease (SCD) vaso-occlusive pain episodes (VOE): Insight from a prospective, multicenter Phase-3 randomized controlled trial

- P3 | "ACSseverity was defined a priori as mild (no oxygen (O2) use or RBC transfusion), moderate (O2 use ortransfusion), or severe (bilevel positive airway pressure (BiPAP) use, intubation, or pediatric intensive careunit (PICU) transfer). 271 patients enrolled (median age 15[11,18] years, 51% male; 74% HbSS/Sb°; 76% onHydroxyurea (HU)...Across all sites, 96% of patients receivedantibiotics (50% ceftriaxone, 28% ampicillin-sulbactam, 7% vancomycin, 11% other) and 82% receivedazithromycin. Site-level use ranged from 0-100% for ceftriaxone and 50-100% for azithromycin... ACS remains common in patients with SCD-VOE (20%), with 14% of ACS diagnosed duringtheir hospital stay. Two-thirds of subjects who developed ACS had a CXR done in the ED that wasnegative. ACS may have been missed in patients who did not get a CXR, and radiographic changes oftenlag behind symptoms."

Nanocurcumin: An Innovative Strategy to Combat Virulence and Methicillin Resistance in Staphylococcus aureus Isolated from Burn Wounds.

- "The 50 selected isolates were resistant to cefoxitin, amoxicillin, benzylpenicillin, ampicillin/sulbactam, piperacillin/tazobactam, cloxacillin, oxacillin, and azithromycin. No significant changes were seen in fnbA and ftsZ. Nanocurcumin inhibits S. aureus virulence and resistance genes, reducing biofilm formation and toxin production, but lacks effect on fnbA and ftsZ, requiring further research."

- "Outpatient urinary antigen tests for pneumococcus and Legionella were negative, and oral garenoxacin was initiated...Broad-spectrum β-lactams (sulbactam/ampicillin, followed by piperacillin/tazobactam) were ineffective...Subsequent targeted therapy with azithromycin (days 6-8 and 13-15), alongside levofloxacin, resulted in clinical improvement...This case report highlights that a single negative urinary antigen test does not rule out Legionella infection. We accordingly recommend early repeat antigen testing, ideally within one week, for postoperative pneumonia unresponsive to β-lactams, together with prompt macrolide or fluoroquinolone therapy."

Stronger implementation of national standards is essential to reduce overtreatment and support antimicrobial stewardship."

- "Initial empirical therapy consisted of meropenem and vancomycin, subsequently adjusted according to the susceptibility profile to cefiderocol, intravenous and intrathecal colistin, combined with ampicillin/sulbactam and minocycline. Intrathecal administration of colistin, in combination with systemic multidrug therapy, proved decisive in controlling the infection. An interdisciplinary approach and individualized antimicrobial regimens are essential to achieving a favorable prognosis in such complex cases."

Bacillus mycoides bacteremia after dog bite trauma in a patient on chronic suppressive antibiotics.

- "The patient, who had a chronic Left Ventricular Assist Device (LVAD) in place and was on a chronic suppressive regimen of doxycycline and cefadroxil for a preexisting driveline infection, developed B. mycoides bacteremia despite ongoing antibiotic therapy. She was successfully treated with intravenous ampicillin-sulbactam plus vancomycin, followed by oral ciprofloxacin upon discharge. The patient's recovery was uncomplicated and the final susceptibility testing confirmed pan-susceptibility to all tested agents. This case highlights the potential pathogenicity of B. mycoides in humans and raises questions on the risk factors for infection and optimal management of B. mycoides infections in the setting of relatively limited clinical data."

- "According to all culture data, the rate of antibiotic susceptibility was third- and fourth-generation cephalosporins between 76.4 and 68%, 68% for ampicillin-sulbactam, 81.8% for piperacillin-tazobactam, 66% for ciprofloxacin, 71% for levofloxacin, 30% for tetracycline, and 45% for penicillin. The increase in Gram-positive infections and resistance to other antibiotic groups indicates empirical antibiotic selection based on local microbial culture results and antibiotic susceptibility. Our results are crucial to allowing for more rational and successful use of antibiotics."

Suspected Tazobactam Allergy in A Case of Anaphylaxis in Multiple Drug Allergy

- "The patient's listed allergies included: penicillin, piperacillin-tazobactam, and multiple cephalosporin. Penicillin resulted in anaphylaxis 9 years prior, as well as anaphylaxis to ceftriaxone and cefepime. Penicillin skin testing was negative and he passed 2-step amoxicillin challenge. He subsequently tolerated Ampicillin-sulbactam. Based off susceptibilities, the primary team requested to change therapy to ceftolozane-tazobactam...He was treated for anaphylaxis with intramuscular epinephrine. Tryptase during event was 13.4μg/L. Discussion This case highlights the importance of history in drug allergy evaluation, as well as consideration of tazobactam as a separate allergenic agent."
