He was evaluated at an urgent care and discharge with ketorolac and a prescription for ondansetron...He was treated with Lorazepam and Dexmedetomidine...This case illustrates the importance of thorough history-taking and exam and awareness of drug-induced neurologic syndromes in critically ill patients. Early recognition of serotonin syndrome can significantly alter outcomes."

A Novel Approach to Estimating the Risk of Gastrointestinal Bleeding for Patients on Oral Anticoagulants and Other Medications Including Non-Steroidal Anti-Inflammatory Drugs and Antidepressants

- " Risk of GIB for OAC products obtained from pivotal studies were: 1.99% for apixaban; 3.15% for rivaroxaban; 2.99% for dabigatran; 3.31% for edoxaban; and 2.00% for warfarin. The odds of GIB for NSAIDs ranged from 1.16 (95% CI:0.84-1.61) for celecoxib to 20.67 (95% CI:14.56-29.34) for ketorolac. Odd ratios for GIB among SSRI/SNRI ranged from 1.31 (95% CI:1.07-1.62) for paroxetine to 1.50 (95% CI:1.32-1.70) for venlafaxine. Risk of GIB for other factors included: persons over 65 years of age (OR=2.50, 95% CI:1.2–5.5); previous history of GIB (OR=5.13, 95% CI:3.95–6.67); aspirin use (OR=1.31, 95% CI:0.93-1.85); antiplatelets use (OR=1.95, 95% CI:1.68-2.27); and corticosteroids use (OR=1.68, 95% CI:1.49-1.90)... The DIOAC-GIB incorporates drug interactions with OACs and had good correlation with other bleeding risk scores. DIOAC-GIB may help identify high-risk patients that benefit from tailored prescribing. - "The lesion was extensive, nearly circumferential, and severely impaired quality of life; the patient required a cane for ambulation and ingested up to ten tablets of ketorolac daily for pain control...Corticosteroid therapy was initiated with topical agents and systemic prednisone, starting at 50 mg/day and tapered to 25 mg/day...It also illustrates how timely recognition of pyoderma gangrenosum, after thorough exclusion of infection and malignancy, can dramatically alter prognosis. - "All patients received standard anesthesia and postoperative ketorolac with morphine PCA. Four patients (13%) in the magnesium group had transient hypotension; no serious adverse events occurred. Low-dose intravenous magnesium sulfate safely reduced pain and opioid needs in orthopedic surgery and may be considered in multimodal analgesia strategies."

Safety and Efficacy of Ketorolac in the Management of Pain for Postoperative Pediatric General Surgery Patients.

- "Logistic regression measured odds of opioid use in morphine equivalents 24-hour after surgery.Results5455 patients were identified. Blood transfusions were less common in patients that received Ketorolac (0.2% vs 0.7%, P = 0.005).ConclusionKetorolac does not increase the risk of bleeding requiring transfusion and decreases opioid administration. Ketorolac is efficacious in this patient population either alone or in combination with opioids and improves pain control compared to opioids alone."

- "To determine the safety of an intracanalicular dexamethasone ophthalmic insert (IDOI) and topical Ketorolac tromethamine 0.5% (ketorolac) versus ketorolac and topical prednisolone acetate 1% (PA) over the first month after cataract surgery (POM1). There was no difference in the rate of rebound inflammation or CME in the IDOI/ketorolac regimen compared to the PA/ketorolac regimen. IDOI can be a safe and effective dropless alternative to PA/ketorolac therapy after cataract surgery in patients susceptible to inflammation."

Binimetinib and cystoid macular edema: a therapeutic dilemma in patients with metastatic melanoma.

- "She was initially treated with Vemurafenib and Cobimetinib, achieving complete remission. However, due to cumulative toxicities, therapy was switched to Encorafenib and Binimetinib in February 2023...Management included Binimetinib dose reduction and topical ketorolac, resulting in initial improvement, although the CME recurred several months later...The patient ultimately began immunotherapy with Nivolumab and Ipilimumab, but died in February 2024 due to refractory abdominal septic shock. This case highlights the importance of early ophthalmologic monitoring and interdisciplinary collaboration in patients receiving MEK inhibitors."

- "The use of ketorolac, meloxicam, diclofenac, celecoxib, and ibuprofen significantly increased with time while the use of naproxen, nabumetone, etodolac, sulindac, piroxicam, and indomethacin significantly decreased (Table 2). Outpatient NSAID prescriptions by orthopaedic surgery providers increased between 2013 and 2021and is likely underestimated by these data as the majority of NSAIDs are available as over the counter medications. Further study is needed to identify if these practice patterns represent rising use in the setting of fracture surgery as historically this has been the subspecialty where their use was most limited."

- "One hundred patients with isolated, unilateral, chronic ACL tears were randomized 1:1 to receive either an intra-articular injection containing TXA (1 g), morphine sulfate, ketorolac, lidocaine, and normal saline (intervention group) or an identical injection without TXA (control group). Intra-articular TXA did not significantly reduce early postoperative pain or opioid use when added to a multimodal analgesic regimen and intravenous TXA in arthroscopic ACL reconstruction. However, it was associated with slightly better functional outcomes at 1 and 3 months postoperatively, as measured by KOOS and Lysholm scores. Although its clinical effect is questionable."

Efficacy of Local Infiltration Anesthesia versus Interscalene Brachial Plexus Block for Postoperative Pain Control after Arthroscopic Rotator Cuff Surgery: A Noninferiority Trial

- " Patients undergoing arthroscopic rotator cuff surgery were randomized to receive either ISB (20 ml of 0.375% bupivacaine) or LIA administered to the subacromial space and arthroscopic portal incisions (20 ml of 0.5% bupivacaine, 1 ml of ketorolac [30 mg/ml], and 19 ml of normal saline)...Additionally, cumulative morphine consumption at 24 hours was significantly greater in the LIA group compared to the ISB group (20.2 ± 17.1 mg vs. 9.0 ± 6.3 mg; p = 0.02), as was intraoperative fentanyl requirement (171.8 ± 54.4 mcg vs. 131.3 ± 38.9 mcg; p < 0.001)... Local infiltration anesthesia (LIA) was non-inferior to interscalene brachial plexus block (ISB) for pain control at 24 hours after arthroscopic rotator cuff surgery. However, ISB provided superior analgesia during the immediate postoperative period (0–12 hours) and was associated with significantly lower intraoperative and postoperative opioid consumption."

Intraoperative Methadone Versus Regional and Local Anesthesia in Periacetabular Osteotomy with Hip Arthroscopy: Postoperative Pain, Opioid Use, and Length of Surgery in Three Cohorts

- "This is the first study to evaluate methadone use in PAO with arthroscopy, comparing efficacy and opioid consumption of three different anesthetic regimens: (1) lumbar plexus catheter with a single shot intrathecal hydromorphone (LP+HM) (2) locally administered intraoperative soft tissue periarticular injection with ropivacaine, epinephrine, and ketorolac (arthroplasty block [AP]), and (3) AP with intraoperative methadone (AP+M)...Primary outcomes were 48-hour morphine milligram equivalents (MME) and total inpatient opioid consumption... Intraoperative methadone added to the arthroplasty block provided comparable analgesia and reduced total opioid use compared to arthroplasty block alone. These findings support methadone as an adjunct to periarticular anesthesia for improving pain control and reducing opioid use in hip preservation surgery."

Among patients undergoing PAO, the combination of a periarticular injection and decreased recommended number of discharge opioids was associated with significantly reduced opioids prescribed at discharge and in total compared to epidural catheter, yet had a higher incidence of early refills. These findings support the role of a PI with ropivacaine in opioid minimization strategies but highlight the need for tailored discharge planning to ensure adequate pain control."

Does adding corticosteroids to the cocktail solution for Local Infiltration Analgesia in TKA minimise knee inflammation? A randomised controlled trial

- "The CF-LIA mixture consisted of 20 mL of 0.5% bupivacaine, 0.3 mL of 1:1000 adrenaline, 30 mg of ketorolac, and 0.9% saline solution, for a final volume of 40 mL. The C-LIA mixture consisted of similar agents with an additional 40 mg of Solumedrol... The significant increase in the levels of inflammatory cytokines in synovial fluid confirmed the inflammation after TKA in both C-LIA and CF-LIA groups. However, although studies have shown that adding corticosteroids to the cocktail solution decreases postoperative pain and accelerates functional recovery, the present study found that it did not minimize the level of knee inflammation."

This review found 102 complications across 25 RCTs that compared a PRP injection to an alternative injection for treatment of rotator cuff tendinopathy. The pooled complication rates for the PRP group were statistically similar to complication rates of saline and steroid injections. There were no reports of serious events across al l studies."
