Targeting the carotid body with Big-K+-channel blocker ENA-001 in drug-induced respiratory depression (PAINWeek 2026) - "In mice, rats, and non-human primates, ENA-001 increased ventilation and attenuated respiratory depression induced by morphine and by xylazine–fentanyl...In experimental models of alfentanil- and propofol-induced respiratory depression, ENA-001 significantly improved isohypercapnic minute ventilation and fully restored the hypoxic ventilatory response, respectively, without impairing sedation or analgesia... The initial search resulted in 611 papers. Eight relevant publications were identified, four reporting human data. ENA-001 acts at the carotid bodies, which serve as broad-spectrum metabolic sensors detecting hypoxia, hypercapnia, acidosis, hypoglycemia, hyperthermia, catecholamines, selected hormones, and inflammatory signals."

Effect of Lidocaine Spray on the Sedative Dose of Remimazolam During Gastroscopy in Adults: A Prospective, Randomized Dose-Response Trial Using an Up-Down Sequential Allocation Method. (PubMed, Drug Des Devel Ther) - "At the start of sedation, alfentanil was intravenously injected at a dose of 3 μg/kg, followed by remimazolam administration using a sequential method. In addition, no statistically significant differences were observed in hemodynamic changes or related adverse events between the two groups (P > 0.05). Lidocaine spray can be used in combination with remimazolam for gastroscopic examination to reduce the sedative dose of remimazolam during gastroscopy in adults."

Opioid type, rather than liver function, is associated with postoperative nausea and vomiting after painless gastroscopy in decompensated cirrhosis. (PubMed, Front Pharmacol) - "In this exploratory post-hoc secondary analysis of a double-blind randomized controlled trial, we aimed to determine whether preoperative albumin, total bilirubin, Apfel score, and opioid type (alfentanil vs. nalbuphine) independently predict PONV within 48 h after painless gastroscopy in decompensated cirrhosis, and to compare their effects on 24 h postoperative liver function...Postoperative liver function changes were mild, similar between groups and unrelated to PONV. Our findings are hypothesis-generating and require confirmation in larger studies."
