Intraductal digestive enzyme accumulation drives lethal hemorrhagic necrotizing pancreatitis and enables therapeutic intervention.
(PubMed, Res Sq)
- "We tested a repurposed strategy using clinically approved agents for other indications: secretin to stimulate ductal fluid secretion and isosorbide mononitrate (ISMN) to promote ductal outflow via sphincter of Oddi relaxation... Pancreatitis severity is determined not only by the magnitude of enzyme activation but also by where enzyme activity is exerted, implicating intraductal enzyme accumulation as a mechanistic determinant and therapeutic target in HNP. Coordinated enhancement of ductal secretion and outflow with clinically approved agents prevents HNP in vivo, supporting a translatable prevention strategy."

CORONARY VASOSPASM AND GASTROENTERITIS IN SETTING OF IMMUNE CHECKPOINT INHIBITOR TREATMENT - CASE REPORT
(ACC 2026)
- "Case: The patient is a 58-year-old male with history of metastatic renal cell carcinoma, treated with ipilimumab and nivolumab, who presented with several weeks of progressive diarrhea...He required high dose steroids and infliximab, followed by upadacitinib and vedolizumab, with only limited response to therapy...Patient was started on amlodipine and isosorbide mononitrate with good symptom control and resolution of echocardiographic wall motion abnormalities... Our case highlights that coronary involvement can represent a form of ICI-related toxicity, potentially occurring in conjunction with other organ system adverse effects."

A SHOCKING DECISION: REASSESSING ICD NECESSITY IN HIGH-RISK VASOSPASM-INDUCED VF
(ACC 2026)
- "He was treated with isosorbide mononitrate and diltiazem, alongside lifestyle modifications, though he continued to smoke...After intracoronary nitroglycerin, he developed VF requiring defibrillation... Refined validated risk stratification methods to guide ICD management in vasospasm-induced VF. Strict adherence to vasodilators and lifestyle changes may reduce recurrence risk and could influence long-term ICD decision-making in select patients."

A TOXIC TRIGGER: 5-FLUOROURACIL INDUCED STEMI
(ACC 2026)
- "Decision-Making: Intra-coronary nitroglycerin was given, leading to complete reversal, thereby confirming vasospasm...The patient was treated with verapamil and isosorbide mononitrate... 5-FU induced STEMI is a rare but serious complication arising from coronary vasospasm due to endothelial dysfunction. Prompt recognition is essential, as management differs from atherothrombotic STEMI and relies on discontinuation of 5-FU with vasodilator therapy, thereby avoiding unnecessary percutaneous coronary interventions."

Comparative cardioprotective effects of Kuanxiong Aerosol and its individual components in a rat model of acute myocardial infarction.
(PubMed, Pharm Biol)
- "Animals were pretreated with low-dose (120 µL/kg) or high-dose (200 µL/kg) KXA, its individual components (120 µL/kg), or isosorbide mononitrate (ISMN; 5 mg/kg)...This study provides evidence supporting the cardioprotective potential of low-dose, multi-component KXA, indicating that such balanced formulations may offer broader benefits than high-dose or single-agent approaches. Further research is needed to elucidate the molecular mechanisms and refine the formulation for clinical translation."

Trends in Out-of-Pocket Spending for Drugs Under Medicare Part D in Coronary Artery Disease and the Impact of the Inflation Reduction Act
(AHA 2025)
- "C1 included ticagrelor, isosorbide mononitrate, and atorvastatin; C2 is C1 + ezetimibe; C3 is C2 +bempedoic acid; C4 is C2 + evolocumab; and C5 is C3 + icosapent ethyl and ranolazine. From 2020–2024, modest cost reductions were observed ( C5: $5,632 to $4,400). After the IRA cap in 2025, costs approached or reached $2,000 for all regimens. However, C1 and C2, previously low-cost, paradoxically saw significant OOP increases (C1: +$795, p = 0.0068; C2: +$878, p = 0.009), while C5 experienced a significant reduction (–$2,366, p = 0.005)."

A DOUBLE-EDGED PILL: ASPIRIN-INDUCED KOUNIS SYNDROME
(CHEST 2025)
- "Coronary angiography showed vasospasm in the proximal left anterior descending (90%) and distal right coronary artery (50%), completely resolving with intracoronary nitroglycerin. She was admitted to the intensive care unit and started on nitroglycerin, clopidogrel (without aspirin), diltiazem, nifedipine, and isosorbide mononitrate. Kounis syndrome should be considered in patients with acute coronary syndrome in the context of an allergic reaction. Early recognition and avoidance of the causative agent are crucial for improving outcomes."

Developing treatments for cerebral small vessel disease: a scoping review of licensed interventions for potential repurposing.
(PubMed, F1000Res)
- "Focussed literature review short-listed ten interventions for review by an independent scientific advisory group; they ranked three as most suitable for immediate testing: metformin, tadalafil and isosorbide mononitrate. The approach could be improved with partial automation, text mining and generative pre-trained transformer approaches which would help manage the large data volumes. Further, our data-driven approach could be combined with genetic or other mechanistic methods to further de-risk future trials."

(ATS 2025)
- "While there are congenital causes, acquired cases from drug or environmental exposures, including benzocaine, phenazopyridine, dapsone, and nitrates/nitrites, are more common...Her current medications included isosorbide mononitrate 30 mg daily, hydralazine, carvedilol, and trimethoprim-sulfamethoxazole 320 mg once daily...In our case, the combination of renal failure, trimethoprim-sulfamethoxazole, and isosorbide mononitrate resulted in clinically significant methemoglobinemia. Cessation of isosorbide mononitrate and treatment with methylene blue resulted in dramatic improvement and allowed continued therapy for pneumocystis with trimethoprim-sulfamethoxazole."
