[VIRTUAL] CMV and C. difficile Co-Infection as a Rare Clinical Clue to the Diagnosis of IBD
(ACG 2020)
- "He completed 21 days of oral vancomycin and was discharged with valganciclovir, prednisone taper, and mesalamine enemas. Early recognition of co-infection can prevent additional treatment for CDI and reduce morbidity and mortality seen with co-infection. Additionally, although the diagnosis of IBD can be challenging in the setting of CMV colitis due to similar histological and endoscopic appearance, initiation of treatment for IBD should be considered as CMV and CDI co-infection may be the initial presentation of IBD."

(ACG 2020)
- " This case showcases a diagnostic dilemma with conflicting clues provided by endoscopic examination and histopathological analysis. To reconcile the response to mesalamine and findings on histopathology and endoscopy, we are entertaining the possibility of a new diagnosis of UC coexisting with low-grade B cell lymphoma vs UC with an incidental clonal population."

[VIRTUAL] LOWER RISK OF RECURRENCE WITH A HIGHER INDUCTION DOSE OF MESALAZINE AND LONGER DURATION OF TREATMENT IN ULCERATIVE COLITIS: RESULTS FROM THE DUTCH, NON-INTERVENTIONAL, IMPACT STUDY
(UEGW 2020)
- P=N/A | "A higher induction dose of mesalazine (≥4g/day) and longer duration of treatment (>6 months) was associated with a lower risk of recurrence. Results from previous studies in UC suggesting better outcomes with a longer induction period are therefore - for the first time - confirmed in a non-interventional, real world evidence study."

A novel rat model of inflammatory bowel disease developed using a device created with a 3D printer.
(PubMed, Regen Ther)
- "We evaluated the effects of conventional drug therapies (mesalazine and prednisolone) and local transplantation of allogeneic adipose-derived mesenchymal stem cells (ASCs) on ulcer size (measured from photographic images using image analysis software) and degree of inflammation (assessed histologically). The new model of TNBS-induced colitis, made with the aid of a device fabricated by 3D printing, generated ulcers that were reproducible in size. We anticipate that our new model of colitis will provide more reliable measures of treatment effects and prove useful in future studies of IBD therapies."

Successful Use of Combination Biologic Therapy in Medically Refractory Pediatric Crohn's Disease and Sacroiliitis
(AIBD 2019)
- "He had sub-optimal clinical improvement on anti-tumor necrosis factor (TNF) therapy that included infliximab for 1 year, followed by adalimumab for 1.5 years...At the time of presentation to our center he was steroid dependent on prednisone 5 mg daily for 6 months while still on vedolizumab and azathioprine...He was treated with Budesonide MMX and mesalamine enemas with no improvement...Adalimumab and oral methotrexate were given in addition to vedolizumab...When used judiciously in patients with medically refractory disease it can be safe and effective. A paucity of literature exists regarding the use of combination biologicals especially in children."

"Crystal Crater" Sevelamer-Induced Rectal Ulcer: A Rare Cause of Lower Gastrointestinal Bleeding from a Common Medication
(APDW 2019)
- "Sevelamer carbonate is a widely available and used medication for the treatment of hyperphosphatemia in patients with chronic kidney disease. As of the time of this case report, approximately 6 months after, hematochezia has not yet recurred. Though sevelamer induced rectal ulcers causing gastrointestinal bleeding are rare, attending nephrologists, gastroenterologists and pathologist should be aware of the possibility of the drug's effects as well as the typical morphological features on biopsy to establish the diagnosis - which would lead to prompt recognition and discontinuation of the offending drug."

Uncharted Territory: Biologic Therapies for Elderly Patients With Refractory Ulcerative Colitis
(ACG 2019)
- "She was then switched to hydrocortisone enemas, then switched to adalimumab, with resultant persistent recurrence of flares. She is now on infliximab and in remission since 2018...Advantages of this therapy are low rates of drug interactions and lesser impact on comorbidities when compared with usual treatments like steroids or 6-MP. Our experience supports the use of biologics in elderly patients with refractory UC."

SORBICILLIN FRACTION OF PENICILLIUM FLAVIGENUM FOR THE TREATMENT OF INFLAMMATORY BOWEL DISEASE: IN VITRO AND IN VIVO STUDIES
(UEGW 2019)
- "Sorbicillin-containing aqueous fraction of Penicillium flavigenum does not have cytotoxic and apoptotic effects on Caco-2 cell, but has anti-inflammatory effect by reducing IL-1β levels in LPS-stimulated THP1 cells. Sorbicillin ameliorates iodoacetamide-induced UC in rats. The restoration of mucosal barrier and resolving inflammation are the potential mechanism of sorbicillin beneficial effect."

Effect of Shaoyao Tang on ulcerative colitis in rats via regulation of TLR4/NF-κB signal pathway
(PubMed, Zhongguo Zhong Yao Za Zhi)
- "Immunohistochemistry and Western blot assay showed that the levels of TLR4 and NF-κB protein expression in the colon tissues of Shaoyao Tang high-dose group were remarkably lower than that in the model group(P<0.001). This study shows that Shaoyao Tang has protective and repairing effects on UC, and its possible mechanism is achieved probably by regulating the TLR4/NF-κB pathway and inhibiting the expressions of MPO, P-selectin, MIF and TXB_2."

Crohn's disease in a developing African mission hospital: a case report.
(PubMed, J Med Case Rep)
- "Crohn's disease is uncommon in Africa, and this entity is encountered sparingly. The signs and symptoms of Crohn's disease overlap with many other abdominal disorders, such as tuberculosis, ulcerative colitis, irritable bowel syndrome, and others. Several publications in the literature describe that it is difficult to make an accurate diagnosis of this disease, despite the fact that many diagnostic armamentaria are available to suggest its presence. Most of the patients with Crohn's disease are treated conservatively, and a few may require surgical intervention, especially those presenting with complications such as intestinal obstruction, perforations, and abscess as well as fistula formations, as seen in this index patient. Crohn's disease is considered by many to be a very rare disease in Africa. It is interesting to know that Crohn's disease, which affects mainly young adults, may debut at any age. The rarity and clinical curiosity of this entity..."
