Treatment for Mild Asthma: What Matters to Providers

- "Specialists were more likely to utilize MART therapy than generalists (budesonide/formoterol: 62.5% vs. 60.0% p 0.002, mometasone/formoterol 58.5% vs. 34.8% p 0.019). Similarly, specialists were more likely to prescribe budesonide-albuterol for reliever therapy than generalists (20.1% vs. 4.5%, p<0.001), and attendings more than trainees (17.5% vs. 6.2%, p 0.025)...Lastly, specialists are more likely to utilize asthma action plans (63.5% vs. 43.4%, p 0.030). Conclusion Significant heterogeneity exists in the definition and treatment of mild asthma."

- "The obstructive component of his lung disease was treated with a mometasone-formoterol combination inhaler, montelukast, cetirizine and Dupilumab, an IL-4 and IL-13 pathway inhibitor...In addition, he receives monthly IVIG infusions and bi-monthly inhaled tobramycin...GLILD treatment strategies typically include immunosuppressive agents such as glucocorticoids, rituximab, azathioprine, and mycophenolate mofetil [2]... The necrotizing nature of our patient's granulomas suggests a unique immune dysregulation mechanism, potentially involving defective granuloma resolution and chronic inflammation-driven airway remodeling. Given the absence of a confirmed genetic etiology, further molecular research is warranted to better understand the underlying immune dysfunction."

- "Despite a major paradigm shift in asthma recommendations that occurred in 2019, prescriptions for budesonide-formoterol 160-4.5µg and mometasone-formoterol 100-5µg (which could be congruent with a MART prescription) have not changed. Based on these findings, more efforts are likely needed to improve the adoption of MART in the U.S."

- "Despite treatment with daily high dose inhaled Mometasone/Formoterol and Tiotropium, she continued to have asthma-like symptoms. She started biologic therapy with Dupilumab in August - 2023, which led to only mild improvement of lung function and respiratory symptoms...Subpleural cysts (SPC) are often associated with trisomy 21 and other inherited or acquired conditions due to altered alveolar growth. This case illustrates the importance of having high suspicion and considering atypical causes when standard treatments fail and highlights the need for a multidisciplinary approach for accurate diagnosis and management."

- "Description A 36-year-old woman with a history of mild childhood exercise-induced asthma developed an upper respiratory infection after exposure to sick contacts, after which she developed shortness of breath with no improvement with albuterol, budesonide-formoterol and a short steroid course...She was prescribed daily prednisone, methotrexate, and mometasone-formoterol, which gradually led to improvement in shortness of breath and resolution of serum hypercalcemia...Infliximab was added to her treatment regimen with resolution of hypercalciuria. The patient subsequently had gradual onset of sarcoid arthropathy, worsened with cold exposure and is being considered for hydroxychloroquine...This patient displayed a constellation of unusual findings due to sarcoidosis, including multiple lesions mimicking metastatic neoplasm, diffuse splenic involvement, and hypercalciuria and nephrolithiasis without hypercalcemia. Biopsy is vital for accurate diagnosis and timely management..."

Utilization of Single Maintenance and Reliever Therapy (SMART) for Moderate and Severe Asthma

- "Notably, patients prescribed ICS-formoterol (Symbicort or Dulera) as a baseline controller inhaler were more likely to be prescribed SMART. SMART is under prescribed to patients who qualify, with most patients prescribed SMART concurrently being prescribed a SABA. Patients with private insurance may be more likely to be prescribed SMART than patients with public insurance. Providers may be more likely to prescribe SMART to patients using ICS-formoterol as a controller inhaler compared to other controllers, and these patients may be more open to a SMART prescription."

There are limited reports of anticholinergic therapies in RADS and our success may prove helpful for other clinicians."

Penicillium is a possible cause of ABPM in children and should be considered in treatment-refractory chronic cough."

Central Adrenal Insufficiency from Inhaled Mometasone Manifesting as Growth Deceleration

- "She was treated for pneumonia a week earlier with levofloxacin...The patient has a history of severe persistent asthma and chronic sinusitis on a mometasone-formoterol inhaler for ten years...The bronchoscopy and intraoperative culture revealed methicillin-resistant Staphylococcus aureus (MRSA), and she received an extended course of trimethoprim-sulfamethoxazole...A left-sided chest tube was placed on admission, and meropenem and vancomycin were started empirically...She was started on six weeks of vancomycin and amoxicillin-clavulanic acid therapy... Our case illustrates the rare possibility of bacterial pneumonia and empyema with biologics and the risk of recurrence with continued use, irrespective of the class of biologic used."

Chronic Obstructive Pulmonary Disease Exacerbations and Pneumonia Hospitalizations Among New Users of Combination Maintenance Inhalers.

- "Combination LAMA-LABA inhalers (aclidinium-formoterol, glycopyrronium-formoterol, glycopyrronium-indacaterol, tiotropium-olodaterol, or umeclidinium-vilanterol) and combination ICS-LABA inhalers (budesonide-formoterol, fluticasone-salmeterol, fluticasone-vilanterol, or mometasone-formoterol). These findings were robust across a range of prespecified subgroup and sensitivity analyses. In this cohort study, LAMA-LABA therapy was associated with improved clinical outcomes compared with ICS-LABA therapy, suggesting that LAMA-LABA therapy should be preferred for patients with COPD."

Concomitant Use of Mepolizumab and Ixekizumab in a Patient with Severe Eosinophilic Asthma and Psoriasis: A Case Study

- "She had a longstanding history of asthma and was currently treated with mometasone-formoterol metered-dose inhaler (MDI) 200 mcg/5 mcg 2 inhalations twice daily, tiotropium soft mist inhaler 2.5 mcg 2 inhalations once daily, and salbutamol MDI 100 mcg when necessary. This patient with severe eosinophilic asthma had no side effects after the addition ofmepolizumab. To the author’s knowledge, this is the first published report of concurrent mepolizumab and ixekizumab therapies."
