We have demonstrated the prevalence of clinically relevant high-risk PGx phenotypes in a diverse paediatric oncology cohort. Many supportive care medications prescribed for paediatric HSCT patients are likely to be affected, with high prescribing rates observed for some medications with available PGx prescribing guidelines. These results highlight the potential for PGx guided prescribing to improve medication efficacy and reduce toxicity in a paediatric HSCT setting."

Antibiotic use and gut microbiome composition links from individual-level prescription data of 14,979 individuals.

- "Clindamycin, fluoroquinolones and flucloxacillin accounted for most of the associations with the abundance of individual species. Use of these antibiotics 4-8 years earlier was associated with altered abundance of 10-15% of the species studied; penicillin V, extended-spectrum penicillins and nitrofurantoin were associated with only a few species. Similar results were found comparing one antibiotic course 4-8 years before sampling versus none in the past 8 years. These findings indicate that antibiotics may have long-lasting consequences for the gut microbiome."

Functionally, cloxacillin induced oxidative stress, mitochondrial dysfunction, and activation of apoptotic signaling pathways. Overall, the collagen hydrogel-based 3D HepaRG model provided a robust and physiologically relevant platform for investigating antibiotic-induced cholestatic DILI and its underlying mechanisms, offering improved predictivity over traditional 2D systems and valuable potential for preclinical drug safety assessment."

- "Six antibiotics typically used in PJIs were selected for testing: vancomycin, gentamicin, flucloxacillin, rifampicin, cefuroxime, and cotrimoxazole. We demonstrate that in organisms typical for PJI, SLIC detects infection rapidly, shows a high PPV for the presence of antimicrobial resistance, and has strong concordance with BMD in defining AST, at 60 and 120 minutes. This timescale compares favourably to the current laboratory standard (culture and colorimetry), which typically returns results in 48h or longer. SLIC also obviates the need for a priori knowledge pertaining to the likely organism, as is often required by rapid molecular methods."

- "Recent years have seen a notable increase in the number of publications concerning pharmacokinetic drug-drug interactions involving the isoxazolyl penicillins cloxacillin, dicloxacillin, flucloxacillin and oxacillin. Although pointing towards induction of cytochrome P450 enzymes, diphosphate-glucuronosyltransferase enzymes and P-glycoprotein as most plausible mechanism for most interactions, the exact interaction mechanisms could not be fully elucidated due to the quality of the study designs and heterogeneity in endpoints. Clinicians should be aware of a potential clinically relevant interaction when combining isoxazolyl penicillins with drugs with a small therapeutic window that undergo extensive metabolism."

An Unusual Case of 5-Oxoproline Acidosis.

- "Although there have been plenty of case reports of this condition because of the coadministration of paracetamol/acetaminophen and flucloxacillin/oxacillin, to our knowledge, this is only the second published case of 5-oxoproline acidosis without paracetamol use, making it either highly unusual or potentially underdiagnosed. Therefore, we suggest considering 5-oxoproline/pyroglutamic acidosis when evaluating the etiology of metabolic acidosis in patients on high-dose flucloxacillin/oxacillin, even without a history of concomitant or previous paracetamol exposure or other classical risk factors."
