Topical Diltiazem in the Management of Cutaneous Leiomyoma-Associated Pain: Evaluation With a Cold-Provocation Test.
(PubMed, Australas J Dermatol)
- "We report a patient with genetically confirmed hereditary leiomyomatosis and renal cell cancer (HLRCC) in whom topical diltiazem 2% was associated with reduced pain, assessed using a controlled cold-provocation test and validated pain scores. This brief report suggests that topical diltiazem may be a useful symptomatic option and supports further evaluation."

Comparative Efficacy and Safety of Vasodilators Used to Prevent Internal Thoracic Artery Spasm in Coronary Artery Bypass Grafting: A Systematic Review.
(PubMed, Cureus)
- "This review systematically evaluates the comparative efficacy and safety of vasodilators used to prevent ITA spasm, including papaverine, glyceryl trinitrate-verapamil (GV) solution, systemic diltiazem, and efonidipine. Delivery method influenced efficacy, with topical or intraluminal administration providing rapid, direct vasodilation. Overall, combination therapy (GV solution) offers a practical and safe approach for intraoperative ITA spasm prevention."

Hypotensive Agents for Controlled Hypotension in Endoscopic Sinus Surgery: A Systematic Review and Network Meta-Analysis.
(PubMed, JAMA Otolaryngol Head Neck Surg)
- "Randomized clinical trials of patients undergoing ESS and receiving dexmedetomidine, clonidine, esmolol, labetalol, bisoprolol, metoprolol, diltiazem, nifedipine, or verapamil vs placebo. In this systematic review and network meta-analysis, controlled hypotension improved surgical field quality in ESS with comparable safety. Diltiazem, esmolol, dexmedetomidine, labetalol, and clonidine were effective; dexmedetomidine was associated with reduced MAP the most, and esmolol favored recovery."

(PubMed, Ugeskr Laeger)
- "This case report presents a 44-year-old woman, who was assessed by several different specialities, due to persistent perianal symptoms, while treated intermittently with diltiazem gel for an anal fissure. After 18 months, the patient was diagnosed with complicating allergic contact dermatitis caused by diltiazem gel."

Na+ Uptake Triggered in Adrenal Chromaffin Cells by a 5-ns Electric Pulse is Facilitated by Ca2+ Influx Via an L-Type-Like Ca2+ Channel Independent of Changes in Membrane Potential.
(PubMed, J Membr Biol)
- "A 5-ns, 5 MV pulse elicited a tetrodotoxin-insensitive rise in ING-2 fluorescence that exhibited a similar electric field dependency and response to pulse-pair stimulation as the inward current...Full inhibition was achieved with the selective L-type voltage-gated Ca2+ channel (VGCC) inhibitors nitrendipine, verapamil, or diltiazem but not the broad-spectrum inhibitor Cd2+...These results provide evidence that nanoelectropulse-induced Na+ influx into chromaffin cells involves several Na+ influx pathways that are facilitated by Ca2+ influx via an L-type-like Ca2+ channel associated with cholesterol-rich membrane domains. Investigating such membrane effects is essential for developing nanosecond electric pulse technologies for stimulating and/or modulating excitable cells."

Sudden Death With Vasospastic Angina That Could Not Be Medically Managed.
(PubMed, Cureus)
- "During admission, telemetry captured a prolonged ventricular pause (~10 seconds) secondary to diltiazem...This case illustrates a rare but severe form of vasospastic angina that remained constrained and refractory to escalating medical therapy and ultimately resulted in a fatal ventricular arrhythmia. Early recognition, optimization of vasodilator therapy, and careful rhythm surveillance are essential, but even with appropriate management, some patients remain at risk for sudden cardiac death."

Misdiagnosis of Supraventricular Tachycardia as Panic Attacks: A Case Report Highlighting the Importance of Accurate Cardiovascular Evaluation.
(PubMed, Cureus)
- "The patient was successfully treated with intravenous diltiazem, and her symptoms resolved promptly...Timely and accurate diagnosis is crucial to avoid delays in appropriate treatment and prevent unnecessary psychiatric interventions. The case highlights the value of thorough cardiovascular evaluation, particularly in patients with recurrent or unexplained episodes of palpitations, as well as the role of advanced diagnostic techniques in improving diagnostic accuracy."

Using Physiologically Based Pharmacokinetic Modeling and Simulations to Predict Antihypertensive Drug Doses in Cirrhotic Patients.
(PubMed, J Clin Pharmacol)
- "The model predicted that the healthy doses of nifedipine 20 mg three times daily (TID), verapamil 80 mg TID, nebivolol 10 mg once daily, and diltiazem 60 mg TID should be adjusted to 44%, 41%, 49.8%, and 51% of these doses, respectively, in the mild cirrhosis population. Among the studied drugs, nifedipine was the most affected and diltiazem was the least affected by cirrhosis, highlighting variability in hepatic impact across antihypertensive drugs. In the absence of dedicated clinical trials in cirrhosis for these drugs, validated PBPK models offer evidence-based insights to support clinicians in evaluating antihypertensive dosing options."

Evaluation of Surgical and Conservative Approaches in the Treatment of Anal Fissure: A Systematic Review.
(PubMed, Acta Gastroenterol Belg)
- "Conservative methods - particularly topical diltiazem and nifedipine, botulinum toxin injections, and pelvic floor therapy - were effective in pain reduction and healing for many patients and had mostly mild adverse effects. Both surgical and conservative approaches serve as complementary strategies in the management of anal fissures. Surgical methods, particularly LIS, provide an effective treatment option with high success rates, while conservative approaches are beneficial in the early stages and for specific patient populations."

Hydrophobic hydration of analgesics and diltiazem complexes explored by electrochemical impedance spectroscopy and diffusion-ordered spectroscopy.
(PubMed, Phys Chem Chem Phys)
- "Analgesics and DTZ were dispersed as free entities in deuterated dimethyl sulfoxide (an aprotic solvent), whereas oligomeric clathrates were observed in heavy water (a protic solvent). These findings suggest that the hydrophobic hydration of the complex formed by acidic and basic drugs may involve intermolecular electrostatic interactions embedded within the hydrogen-bonding network in water."

Capillary blood microsampling and LC-Orbitrap analysis for adherence monitoring of cardiovascular drugs: method development, cross-validation, and patient proof-of-concept using VAMS and Capitainer-B.
(PubMed, Talanta)
- "Amlodipine, atenolol, atorvastatin, bisoprolol, carvedilol, clopidogrel, diltiazem, lercanidipine, metoprolol, nebivolol, prasugrel, rosuvastatin, salicylic acid, simvastatin hydroxy acid, and verapamil were included. In a proof-of-concept involving 30 patients, finger-prick concentrations from VAMS and Capitainer-B were interchangeable, supporting the suitability of these adherence workflows but reference ranges for capillary blood remain necessary to enable routine clinical interpretation. In summary, this work established a reliable, LC-Orbitrap-based microsampling platform for monitoring adherence and identified stability issued amongst others that guide future method selection and development."

Of greatest relevance to practice is the effectiveness of: enalapril or amlodipine (vs. nifedipine) in controlling BP; and BP self-measurement/management or lifestyle change (vs. usual care) in preventing longer-term cardiovascular outcomes."
