- "The wound site was left open to promote healing; however, this was complicated by concomitant RDEB resulting in persisting well-demarcated ulceration unresponsive to topical Oleogel-S10, mupirocin 2% and Trimovate®. Botulinum toxin injections, topical glyceryl trinitrate and diltiazem are now being considered as the next step for this patient, with the aim of encouraging skin healing by relaxing the anal sphincter muscle...Additionally, this case presents the importance of tailored examinations under anaesthesia to facilitate perianal diagnoses in patients with EB, where skin fragility and pain limit the feasibility of routine examination methods. This case joins the existing literature that reports the challenges of wound healing in EB postoperatively, but importantly suggests that future management of other, nonsurgical perianal wounds in EB could also utilize similar techniques involving therapeutic anal sphincter muscle relaxation."

- "The results showed that hERG block-mediated QTC prolongation with dofetilide was shortened by co-administration of lidocaine or mexiletine-drugs that inhibit INaL...Consistent with this interpretation, nifedipine, which preferentially inhibits ICaL over hERG (ICaL: 13.2 nM; hERG: 35 μM; ratio = 2,651) caused APD shortening without AP triangulation. Results from this study thus support the following: 1) diltiazem failed to reduce moxifloxacin-induced QTC prolongation due to its concomitant hERG block at clinical exposure levels; and 2) the importance of using physiologically relevant protocols to generate ion channel pharmacology and obtaining functional recordings from myocytes to provide a better understanding of nonclinical data translation to clinical ECG signals. - "Diltiazem was effective and well tolerated in patients with obesity, even at lower actual-body-weight-based doses. Higher initial doses increased the likelihood of early rate control without increasing adverse events, suggesting modestly higher, individualized dosing may be safe and effective. Ideal-body-weight-based dosing better aligned with guideline targets, supporting its use in obese patients. Prospective studies are needed to determine optimal dosing strategies to maximize the efficacy and safety of diltiazem."
