This is hitting the heparin sweet spot: Here is a retrospective analysis of investigating and optimizing aPTT monitoring for safety and efficacy during intravenous heparin infusion
(ASH 2025)
- "Despite a well established protocol, poor compliance, particularly with timely aPTT draws, remained a key barrier. Addressing workflow inefficiencies through interdisciplinary collaboration, better nursing documentation, and EMR optimization may improve adherence and enhance patient safety."

Protamine Overdose–Associated coagulopathy in the setting of post-PCI RCA dissection and cardiac tamponade: A hematologic management dilemma
(ASH 2025)
- "She received 9000 units of IV heparin and was mistakenly administered 250 mg of protamine instead of 50 mg...Antiplatelet therapy (aspirin and clopidogrel), initially held due to bleeding concerns, was successfully re-initiated 24 hours post-intervention... This case underscores the importance of hematologic vigilance in the setting of protamine overdose during cardiac intervention. Accurate dosing, point-of-care coagulation assessment, and delayed—but timely—re-initiation of antiplatelet therapy can mitigate both bleeding and thrombotic risks. Hematologists should be familiar with protamine pharmacodynamics and emerging reversal strategies, as interdisciplinary collaboration is essential in navigating such high-stakes, dual-risk scenarios."

Delayed pulmonary embolism following oocyte retrieval in invitro fertilization: A neglected risk factor for thromboembolic events?
(ASH 2025)
- "Treatment: The patient was managed with intravenous heparin with a subsequent transition to apixaban after stabilization. This case highlights the diagnostic dilemma and management of delayed PE in ART patients. It emphasizes the importance of early recognition and the need for a high index of suspicion for PE, even in the absence of classic risk factors. Prophylactic anticoagulation may be beneficial in ART patients who are at high risk to prevent thromboembolic events, particularly in those with additional risk factors."

Labile heparin response in a pre-operative cardiac surgery patient: Unmasking predisposition for post-operative coagulopathy
(ASH 2025)
- "High-risk patients undergoing cardiac surgery will require IV heparin prior to the procedure...The blood bank can be notified to prepare blood products such as FFP, cryoglobulin or desmopressin which can take longer to obtain in large quantities. This should lead to further evaluation into medications, toxins or biologic causes for the labile response to heparin which may mitigate adverse outcomes. Although the incidence of heparin resistance is rare, performing a clinical study in the future may determine if some of these interventions would be beneficial to patient care."

Elevated aPTT in the setting of prophylactic unfractionated subcutaneous heparin administration
(ASH 2025)
- "Therefore, the patient was given intravenous protamine sulfate 50 mg once and received intravenous 0.6 mcg of desmopressin for concerns of uremic bleeding. Critically ill patients are predisposed to a higher baseline bleeding risk due to factors such as platelet dysfunction, coagulation factor deficiencies, more invasive procedures, and mechanical ventilation. Understanding all these factors is a crucial part of decision-making when administering anticoagulation."

Steroid-sparing use of anakinra for secondary hemophagocytic lymphohistiocytosis in sickle cell disease with multiorgan failure – a case report
(ASH 2025)
- "Management included intubation and ventilation, vasopressor support, and intravenous heparin. This therapeutic choice is supported by emerging evidence favoring IL-1 blockade as a safer immunomodulatory strategy in selected ICU patients with features suggestive of HLH. Our case adds to the growing body of literature advocating for early identification of HLH in SCD patients with unremitting systemic inflammation and organ dysfunction."

Heparin decreases serum sphingosine-1-phosphate levels in patients with vascular diseases.
(PubMed, Atherosclerosis)
- "Circulating S1P concentrations in patients with atherosclerosis are associated to different blood components before and after interventions. Reduced postoperative serum S1P levels in patients are most likely attributed to heparin treatment. The causalities between heparin treatment, reduced serum S1P and reduced intimal hyperplasia deserve further investigations."

Serial Embolic Events From Left Atrial Appendage Thrombus Detected After Mechanical Thrombectomy: A Case Report and Management Considerations.
(PubMed, Cureus)
- "We report the case of an 82-year-old woman with AF on edoxaban who presented with acute right internal carotid artery (ICA) occlusion...Two days after onset, contrast-enhanced chest CT unexpectedly demonstrated an LAA thrombus, prompting initiation of intravenous heparin...Anticoagulation could not be promptly resumed due to hemorrhagic transformation, and the patient subsequently experienced recurrent embolic events. These findings underscore the need for thorough evaluation of potential embolic sources after thrombectomy, thoughtful consideration of when and how to restart anticoagulation, and the possible role of LAA occlusion as a secondary prevention strategy."

Role of heparin-induced HGF release in the acute phase of STEMI.
(PubMed, Front Cardiovasc Med)
- "In a subset of four patients, HGF levels were assessed before and 30 min after heparin injection...This study demonstrates that heparin induces a rapid and substantial increase in circulating HGF in STEMI patients, potentially mediating cardioprotection during reperfusion. These findings suggest that comedications like heparin may confound cardioprotective trials and should be considered in future translational strategies."

Protein-Losing Enteropathy Following Fontan Completion: A 30-Year National Cohort Study in Sweden.
(PubMed, J Thorac Cardiovasc Surg)
- "PLE remains a serious complication following Fontan with notable mortality. Patients with right ventricular morphology of the systemic ventricle had a higher risk of PLE. Previous pulmonary venous congestion was common when PLE was diagnosed early after Fontan. Catheter-based interventions were frequently used to optimize hemodynamics; lymphatic interventions are likely to become more common. In therapy-resistant patients heart transplantation remains an option."

Recurrent Leg Deep Vein Thrombosis in a Female with Atypical Right-Sided Variant of May Thurner Syndrome and Factor V Leiden Mutation
(ASH 2023)
- "She was started on an intravenous heparin drip and subsequently had a thrombectomy procedure. She was subsequently discharged home on apixaban anticoagulation...The patient was then re-initiated on the heparin drip and then bridged to therapeutic warfarin... This is a very atypical presentation of MTS variation affecting the right lower extremity with limited description in the literature. To our knowledge, this is the first case presentation of right sided MTS resulting in a DVT in a patient with factor V Leiden mutation. MTS is a potentially treatable and often-overlooked pathology."

Interaction between Collagen IV and Heparin-Binding Sites in the Activity, Distribution and Clearance of Factor IX(a)
(ASH 2024)
- "Legacy (linear) numbering was used in the Gla domain and chymotrypsinogen numbering in the protease domain. The collagen site had a dominant effect on protease distribution, while the heparin site had similar effects on both zymogen and protease. Disruption of the heparin site modulated the impact of the K5A variant in the protease, and along with the contradictory effects of K5R, suggest potential interaction between these binding sites in the protease."

Safety of early chemoprophylaxis in traumatic brain injury patients.
(PubMed, Surg Neurol Int)
- "A 2-year retrospective cohort study was conducted on eligible trauma patients admitted to MyMichigan Medical Center Saginaw who sustained ICH and received subcutaneous heparin (SQH) prophylaxis within 24 h of the first stable head computed tomography (CTH)...On the first imaging post-SQ heparin initiation, 26 of the 63 patients (41%) had improved ICH, 37 patients (59%) had stable ICH, and none had worsening ICH. In TBI patients with ICH, administration of SQH chemoprophylaxis within the first 24 h of stable imaging appears to be safe, without the risk of worsening hemorrhage."
