- "We report an elderly lymphoma survivor who developed encephalopathy, decompensated heart failure, and a large LV thrombus five years after R-CHOP...He developed AF with RVR (treated acutely with amiodarone)...Given embolic stroke burden, we initiated systemic anticoagulation (IV heparin) and urgent high-dose corticosteroids (prednisone ≈1 mg/kg) to suppress eosinophilic injury... For new LV thrombus with systolic dysfunction and rising eosinophils even years after chemotherapy, demands urgent TTE followed by CMR for tissue characterization to distinguish thrombus from eosinophilic infiltration. Early anticoagulation and prompt immunosuppression (with steroid initiation when safe) are critical to limit embolic complications and irreversible endomyocardial fibrosis; negative marrow/molecular testing supports a secondary HES approach but requires longitudinal surveillance."
