CD21 Things can only get redder! Two cases of allergic contact dermatitis to topical brimonidine tartrate gel.

- "Allergic contact blepharitis to brimonidine eye drops is recognized and, more recently, case reports have described patients developing ACD to topical brimonidine gel [Bangsgaard N, Fischer LAN, Zachariae C. Sensitization to and allergic contact dermatitis caused by Mirvaso® (brimonidine tartrate) for treatment of rosacea - 2 cases...He had been using topical Elocon cream daily (mometasone furoate 0.1%) and was diagnosed with steroid-induced rosacea. He was prescribed oral doxycycline, ivermectin cream and brimonidine gel...We would like to raise awareness that in addition to rebound erythema, topical brimonidine gel is also a cause of severe facial ACD. Patch testing to the patient's own medicaments should be performed in suspected cases."

Vigilant allergen avoidance, individualized treatment plans and education are paramount for optimal management to prevent exposure to such allergens in future treatments."

PA28 Cutaneous sarcoidosis mimicking molluscum contagiosum.

- "In July 2023, dupilumab was commenced by the respiratory team for her severe asthma and it had a beneficial effect on her eczema...Lymecycline 408 mg once daily for 2 months and mometasone furoate 0.1% ointment were commenced but the disease continued to progress. We then commenced doxycycline 100 mg daily alongside mometasone furoate 0.1% ointment due to the patient's strong desire to avoid blood tests...Although it is an uncommon presentation, it is important to be aware of it as a differential diagnosis in cases of atypical molluscum contagiosum. This case, in addition to its severity and rarity, has proven challenging to manage given the patient's severe venepuncture phobia."

- "Topical mometasone furoate ointment combined with an increased dose of oral prednisone (from 5 mg/day to 10 mg/day) resulted in marked improvement within two weeks, followed by complete resolution after several weeks of regular emollient use. Because eczema can closely mimic rejection, it may complicate post-transplant care and lead to unnecessary changes in immunosuppressive therapy if misdiagnosed. Accurate diagnosis is essential for appropriate management and graft survival."

- "Based on clinical and histopathological findings, the patient was diagnosed with napkin psoriasis, and was treated with a mixture of petroleum jelly and mometasone furoate cream 0.1% for two weeks, and pure hydrocortisone 0.1% cream was applied to the face, axillary, and genital areas... This case highlights the importance of considering psoriasis in the differential diagnosis of persistent or treatment- resistant diaper rash, especially when lesions extend beyond the diaper area. Early dermatological assessment and appropriate treatment can lead to rapid and effective disease control, even in infancy. This case also highlights the need for high awareness and timely intervention in pediatric dermatology to ensure optimal patient outcomes."

Trichophyton indotineae dermatophytosis in a patient with Familiar Mediterranean Fever: what a (therapeutic) journey

- "The eruption started approximately 7 months before presentation on his groins and thighs and was initially treated with an over-the-counter cream containing miconazole and hydrocortisone. The lesions continued to spread, and therefore the patient contacted his general practitioner who, following a positive PCR for T. interdigitale, initiated treatment with oral terbinafine at a dose of 250 mg daily and topical mometasone furoate cream...After consulting with the rheumatologist, colchicine treatment was interrupted and oral itraconazole 200 mg daily was initiated, resulting in partial improvement after 4 weeks of treatment...The patient was put on anakinra treatment by the rheumatologist and itraconazole treatment was extended to 3 months as a new culture revealed preserved sensitivity to itraconazole... In terbinafine-resistant dermatophytosis, the challenges seem to go beyond difficulties in species identification, susceptibility testing and identifying clinical..."

Clinical Spectrum of Demodicosis Among Filipinos with Brown Skin: A Case Series

- "Patient used unregulated lightening products containing tretinoin, hydroquinone 2% and clobetasol 0.05% cream for 6 years with minimal improvement. Clinical and histopathological diagnosis revealed rosacea, dyschromia with demodicosis and patient was prescribed low dose isotretinoin and ivermectin 1% cream for the demodex infestation...Another consult done, diagnosed as eczema and was prescribed mometasone furoate 0.1% cream and anti-acne medications with no improvement... Demodicosis is a common skin condition with varying clinical features often underdiagnosed or mistaken for other dermatoses. This case series has documented four cases of demodicosis in patients with diverse clinical manifestations, predisposing factors and therapeutic responses. These findings underscore the importance of early clinical recognition, adequate laboratory investigation and individualized treatment approaches to achieve favorable outcomes."

Due to the rarity of this condition, there is no standardized therapeutic consensus. In this particular case, therapeutic options were limited given the presence of multiple active infections."

Following the diagnosis, topical corticosteroids were discontinued, and the treatment was shifted to tacrolimus and, subsequently, to oral methotrexate with folic acid...It underscores the importance of monitoring systemic side effects in dermatological therapies and educating patients on proper corticosteroid use. Alternative treatments, such as upadacitinib, should be considered in young male patients to prevent severe complications."
