Cardioprotection without risk? meta-analysis of prophylactic agents in low-risk cancer patients on anthracyclines

- "We included randomized controlled trials (RCTs) of prophylactic ACE inhibitors, ARBs, beta-blockers, statins, dexrazoxane, or SGLT2 inhibitors in adults without pre-existing cardiovascular risk factors...Agents studied included beta-blockers (nebivolol, carvedilol, bisoprolol), ACE inhibitors (ramipril), ARBs (telmisartan), and spironolactone. Treatment duration ranged from 4 months to 1 year; anthracycline doses reached up to 689 mg/m² (epirubicin) or 536 mg/m² (doxorubicin)...The protective benefit was greater in contexts involving higher cumulative anthracycline doses and concurrent cardiotoxic therapies such as trastuzumab...However, high heterogeneity limits firm conclusions. Given the moderate effect size and heterogeneity, further large-scale, well-formulated trials are essential to identify optimal cardioprotective agents and precise patient selection criteria."

Pharmacological prevention in cardio-oncology: from bench-to-bedside.

- "Neurohormonal blockers, including ACE inhibitors, ARBs, and β-blockers, constitute the foundation of prevention, although their efficacy varies: combinations such as ACEi/ARB with βB yield mixed outcomes, whereas carvedilol offers antioxidant benefits beyond β-blockade. Sacubitril/valsartan (ARNI) has demonstrated improvements in global longitudinal strain and LVEF preservation in the SARAH trial, albeit with associated hypotension risks. Aldosterone antagonists show potential, with spironolactone preserving LVEF and diastolic function, though eplerenone has not shown significant effects. Statins present conflicting data; the STOP-CA trial supports atorvastatin for LVEF preservation, while the PREVENT and SPARE-HF trials found no benefit. Emerging evidence suggests sodium-glucose cotransporter-2 inhibitors (SGLT2i), such as dapagliflozin and empagliflozin, as promising agents, with preclinical and early clinical data indicating cardioprotection through metabolic..."

Comparison of lipophilic and size-exclusion membranes: creating sink conditions with cyclodextrin.

- "This study aims to investigate the permeability differences of carvedilol between lipophilic (organic solvent) and size-exclusion membranes in the presence of 2-hydroxypropyl-beta-cyclodextrin in just the acceptor compartment or both sides of the membrane using in vitro side-by-side diffusion cell assays...It is also applicable when the solubilizing additive permeates slowly and has minimal influence on transport, such as with a size-exclusion membrane with a 1 kDa molecular weight cut-off. The model remains suitable if the additive is small enough in hydrodynamic size to permeate the membrane, but no concentration gradient exists to drive its transport, for example, with a 6 kDa size-exclusion membrane containing the same CD concentration on both sides of the membrane."

Beta-Blockers as Potential Adjuvants in Melanoma Treatment.

- "The effects of non-selective β-blockers (carvedilol and propranolol), β1 selective blockers (atenolol and metoprolol) and antineoplastics drugs (cisplatin and 5-fluorouracil) on the A375 melanoma cell line were studied, individually and in combined exposures, by assessing cell viability over a 72 h period. Metoprolol and carvedilol exhibited synergistic interactions with cisplatin at specific concentrations. Overall, the data highlight the concentration-dependent nature of mixture effects and support the potential application of β-blockers melanoma treatment."

The incidence of lymphoma among patients with SDS is increased relative to the general population but is lower than the incidence of MDS or AML. Increased toxicity with standard therapies was observed in these patients...."

Home medications included nifedipine and carvedilol...He received induction treatment with rituximab, steroids and avacopan...However, the deposition of both IgG and IgA co-dominant deposits is still rare. In spite of his aggressive RPGN, we elected not to use cyclophosphamide given his recent malignancies."

- "She was started on carvedilol 3.125 mg and low dose Eliquis 2.5 mg twice a day due to high risk for bleeding.Discussion : Factor V Leiden (FVL) results from a point mutation in the F5 gene, which encodes the factor V protein in the coagulation cascade...A study by Kouides et al showed that apixaban has equivalent efficacy in prevention of recurrent VTE but decreased risk of major and minor bleeding events compared with rivaroxaban...Complications can include esophageal variceal bleed and other sequalae of portal hypertension. Multidisciplinary approach is required for individual case including decision about anticoagulation."

Dilated Cardiomyopathy in Adult Propionic Acidemia Improved by Adjustment of Nutrition and Heart Failure Treatment Drugs: A Case Report

- "However, it exacerbated, and he was re- admitted and discharged after introducing carvedilol and pimobendane... In terms of changes before and after heart failure treatment, NTproBNP: 6858→51 pg/ml, chest CTR: 60.7→51.5%, improvement in abnormal electrocardiogram findings, left ventricular end-diastolic diameter (LVDd): 67/60→48/37 mm, left ventricular ejection fraction (EF): 27→51% and improvement in valve regurgitation in echocardiography were observed. The patient showed a favourable 5-year course, diuretic administration was reduced, and cardiac function significantly improved. Controlling acute and chronic heart failure with drugs and readjusting diet therapy according to improved cardiac function allowed the optimisation of the metabolic dynamics and maintain the improved dilated cardiomyopathy."
