Availability and Cost of Basic Drugs for Sickle Cell Disease in 13 African Countries

- "Participants completed questions covering the availability (on a scale from 0 (drug not available) to 10 (no availability concern) and the cost of basic drugs in their centres: HU, penicillin V, folic acid, morphine and vaccines. The high cost of vaccines, beyond the first years of life when a free national program is available in most of African countries, is also a problem. In addition to cost, availability and frequent supply disruptions prevent regular intake of hydroxyurea and, to a lesser extent, of penicillin V. International solidarity and the governments of these countries should work to bring down the cost of hydroxyurea and improve the distribution circuit for these essential medicines."

- "The Cochrane network analysis on the treatment of erythema migrans has only shown that, in addition to doxycycline and amoxicillin, oral penicillin V is equally effective. A Slovenian analysis of patients with acrodermatitis chronica atrophicans over the past 30 years has shown a decrease in the incidence of constitutional symptoms and atrophy, likely due to improved early detection. In conclusion, SA-FAPA-HRMS is considered attractive for rapid and sustainable analysis of pharmaceuticals with potential in non-invasive patient monitoring."

According to the Naranjo Causality Scale, the accountability of both drugs was found to be probable (score of '6'). This patient experienced an AGPE induced by cefazoline and confirmed by a positive IDT. However, where such services are not readily available, the use of cephalosporins empirically is recommended."

- "This study presents a low-cost, cellulose-based solid-phase extraction (SPE) adsorbent (C/PVPP/MDI) for the determination of five antibiotics: tetracycline (TET), ampicillin (AMP), sulfamethoxazole (SMX), penicillin V (PEN V), and chloramphenicol (CAP) in water. Additionally, the C/PVPP/MDI adsorbent was reusable for up to five cycles without significant performance loss and costs approximately 50% less than commercial alternatives. These findings demonstrate the potential of C/PVPP/MDI as a sustainable and affordable SPE material for environmental monitoring of antibiotic contaminants, particularly in developing countries where access to commercial materials is limited."

Crystal structure reveals the hydrophilic R1 group impairs NDM-1-ligand binding via water penetration at L3.

- "This study resolved the crystal structure of NDM-1 hydrolyzing amoxicillin for the first time, revealed that the hydroxyl group in the R1 moiety of amoxicillin anchors a key water molecule (Wat1) via hydrogen bond, inducing a conformational shift in Met67 (average displacement of 3.8 Å compared to its position in complexes with ampicillin, penicillin G, and penicillin V) and impairing the hydrophobic interaction between the loop 3 and the substrate. Steady-state kinetics showed that amoxicillin exhibited a 2.2-fold higher K m and a 5.2-fold higher k cat compared to ampicillin, demonstrating that hydrophilic R1 groups impair enzyme-substrate binding. This work demonstrates the essential role of hydrophobic interactions in L3-mediated substrate binding and provides a novel strategy for designing L3-targeted NDM-1 inhibitors: maximize hydrophobicity and minimize polar surface area in the L3 contact region to block water penetration, thereby stabilizing the inhibitor-L3..."
