Are Antibiotics the Next Class of Psychopharmaceutical Drugs?

- "For example, the isoniazid class of antibiotics has been shown to possess monoamine oxidase activity, while tricyclic antidepressants are reported to have antimalarial and anti-leishmanial properties. Methods: The presentation will review the published literature on antibiotic use in PANDAS and PANS, including: a case series documenting improvement in pediatric OCD following open-label antimicrobial treatment (ML Murphy & Pichichero 2002); improvements in symptom severity associated with open-label monthly Bicillin injections (F Falcini et al, 2013); a placebo-controlled trial of penicillin prophylaxis in PANDAS (Garvey et al, 1999); a comparison of azithromycin and pencillin prophylaxis for PANDAS (Snider et al, 2005); and a randomized placebo-controlled trial of cefdinir for recent-onset PANS (TK Murphy et al, 2015). - "Results of this study suggest that since malarial testing has become more widespread, clinicians working in resource-poor environments still face difficulty when evaluating a child with fever, especially when malaria testing is negative. Improving access to additional diagnostics, continuing medical education, and ongoing evaluation and revision of clinical guidelines may lead to more consistent management of febrile illness by providers, and may potentially decrease prescription of unnecessary antibiotics. Additional interventions at the community level may also have an important role in managing febrile illness, however, more studies are needed to identify targets for intervention at both the clinic and community levels."

- "Pervasive antibiotic misuse was driven by a habitual supplier-seeking behaviour that was enabled by unrestricted access and misconceptions about antibiotics for mild illnesses. Unofficial suppliers must be stopped by supporting existing regulations with tough new laws aimed at outlawing supplies outside registered pharmacies and fining registered pharmacist/owners of these pharmacies for supplying antibiotics without a prescription. Community primary healthcare centres must be strengthened to become the frontline antibiotic prescribers in the community thereby enabling the community's access to inexpensive and appropriate healthcare. Community-based education program should target appropriate health-seeking pathways and the serious consequences of antibiotic misuse."
