Randomized, placebo-controlled trial of N-acetylcysteine in Parkinson's disease patients: a pilot study.

- "This pilot trial evaluated the one-year tolerability of oral N-acetylcysteine (NAC) 1200 mg/day in Parkinson's disease (PD) and explored associations with clinical progression, levodopa dose, and striatal dopamine-transporter imaging. Completer laboratories were unremarkable, but safety cannot be established from completers alone. These data do not establish disease modification and require confirmation in a larger, prospectively powered trial."

Clinical efficacy, safety evaluation, and mechanism of acetylcysteine in improving first-line anti-tuberculosis treatment of newly diagnosed pulmonary TB: an open-label, randomized, single-center trial.

- "The Control group received standard HRZE therapy, whereas the Aerosol NAC and Oral NAC groups received additional nebulized N-acetylcysteine (300 mg twice daily) or oral N-acetylcysteine (600 mg once daily), respectively. The incidence of adverse reactions was lower in both NAC groups (10.0%) than in the Control group (40.0%) (p < 0.05). Adjunctive N-acetylcysteine, administered either by aerosol inhalation or orally, significantly improved treatment outcomes, accelerated sputum conversion, reduced oxidative stress and inflammation, protected liver function, and decreased adverse events in patients with newly diagnosed pulmonary tuberculosis."

- "Methotrexate first addressed the primary inflammatory driver of the itch, followed by oral N-acetylcysteine to target the neuropsychiatric component of onychotillomania, as described in the literature. Persistent nail dystrophy despite treatment, especially in patients with significant psychological distress, should prompt evaluation for onychotillomania, and dermoscopy can be a helpful tool for diagnosis. Clinicians should be alerted to this comorbidity in refractory cases, notwithstanding the small number of cases reported thus far."

ACOX1 Gain-of-Function and De Novo ROBO1 Variant in ACOX1-sEDD.

- "Metabolic testing showed elevated dicarboxylic acids and medium-/long-chain acylcarnitines despite normal very-long-chain fatty acids and classical peroxisomal biomarkers, and trio exome sequencing identified de novo ACOX1 c.710A>G (p.Asn237Ser) and ROBO1 c.572delG (p.Cys191fs) variants. Cutaneous disease resolved with topical 10% N-acetylcysteine and emollients, in conjunction with oral N-acetylcysteine and adjunctive antioxidant vitamin therapy, while ocular involvement improved with vitamin A ointment and autologous serum eyedrops, supporting early recognition of ACOX1-sEDD in infants with inflammatory ichthyosiform dermatitis and ocular-neurologic involvement and suggesting that additional neurodevelopmental variants may modify disease expression."

Our exploratory biomarker findings suggest NAC may be a promising agent to delay MS progression."

- "In one of the recently developed rat’s neurodevelopmental model, schizophrenic-like changes were induced by administration of buthioninesulfoximine (BSO) - an inhibitor of glutathione synthesis and a dopamine reuptake inhibitor - the compound GBR 12909 during the developmental period...The usefulness of the developed model was tested by the effect of the antipsychotic drug aripiprazole and N-acetylcysteine...At the biochemical level, both drugs significantly reduced the elevated concentration of bound sulfane sulfur in the hippocampus of model rats. Recent studies indicate that in the neurodevelopmental pathophysiology of schizophrenia, disturbances in the homeostasis of sulfur compounds play an important role, which are corrected by the action of drugs."

- "This case highlights a patient with chronic liver diseases of HCV and non-alcoholic fatty liver disease (NAFLD) who was started on oral NAC in the setting of non-acetaminophen liver failure.Case Description/ A 54-year-old male with a past medical history of HCV (previously treated/sustained virological response), NAFLD, and recent cocaine/methamphetamine use presented with cough and markedly elevated transaminases (AST 778, ALT 1069). Oral NAC may benefit select patients with HCV and acute liver injury, particularly when IV formulations are unavailable or contraindicated. Further studies are needed to define optimal dosing, timing, and patient selection."
