BI32 Something fishy about that ulcer: a leg ulcer on an immunocompromised scuba diver.
(PubMed, Br J Dermatol)
- "He had a history of psoriatic arthritis, on dual immunosuppression for the past 5 years with methotrexate and adalimumab...He received penicillin antibiotics, topical steroids and mupirocin cream from his general practitioner. He was switched from adalimumab to infliximab for a presumed diagnosis of pyoderma gangrenosum by his treating rheumatology team...His immunosuppression is on hold, both ulcerated lesions continue to slowly enlarge and he remains under close review. This case demonstrates the importance of a full medical and social history, including hobbies and travel, in all patients on immunosuppressive therapy, as they are at risk of atypical infections and presentations."

DS24 Secondary intention healing should be offered for shallow lower-lip defects involving the vermilion border following Mohs micrographic surgery.
(PubMed, Br J Dermatol)
- "Wound care included application of mupirocin or yellow soft paraffin, followed by an initial pressure dressing consisting of a nonadhesive layer, gauze and self-adhesive tape, which was removed after 48 h. Hydrocolloid dressings were then applied and changed three times weekly until full granulation and re-epithelialization were achieved...It offers a balance between minimizing the morbidity associated with complex repairs and achieving acceptable cosmetic and functional outcomes. Appropriate patient selection, shared decision making and meticulous wound care are essential for achieving favourable outcomes."

SG12 A rare case of perianal granular cell tumour in a patient with localized recessive dystrophic epidermolysis bullosa.
(PubMed, Br J Dermatol)
- "The wound site was left open to promote healing; however, this was complicated by concomitant RDEB resulting in persisting well-demarcated ulceration unresponsive to topical Oleogel-S10, mupirocin 2% and Trimovate®. Botulinum toxin injections, topical glyceryl trinitrate and diltiazem are now being considered as the next step for this patient, with the aim of encouraging skin healing by relaxing the anal sphincter muscle...Additionally, this case presents the importance of tailored examinations under anaesthesia to facilitate perianal diagnoses in patients with EB, where skin fragility and pain limit the feasibility of routine examination methods. This case joins the existing literature that reports the challenges of wound healing in EB postoperatively, but importantly suggests that future management of other, nonsurgical perianal wounds in EB could also utilize similar techniques involving therapeutic anal sphincter muscle relaxation."

A Case of Immunotherapy-Induced Balanitis.
(PubMed, Cureus)
- "Although dermatitis can be common, serious dermatologic complications (such as Stevens-Johnson syndrome) have also been reported. In this case report we present a case of balanitis caused by pembrolizumab, briefly review the current literature on more common dermatologic immunotherapy toxicities, and discuss the more common causes and treatments of balanitis. Although drug eruptions commonly respond to discontinuation of the offending agent, here we were able to treat with topical agents, such as steroid cream and mupirocin ointment. The ability to treat the balanitis without needing to discontinue the patient's immunotherapy highlights the success in both controlling the cancer and the treatment-related toxicity."

Prescribing Patterns in Pediatric Cellulitis: A Multi-State Analysis Stratified by Penicillin Allergy Status
(ACAAI 2025)
- "Results Among 1,051,086 pediatric cellulitis patients, cephalexin was the most prescribed antibiotic (n=398,547; 37.92%), followed by mupirocin and amoxicillin/clavulanate. Of 28,871 patients with documented penicillin allergy, cephalexin (n=8,682; 30.07%) remained most prescribed, followed by TMP-SMX, clindamycin, and cefdinir...Conclusions Despite allergy labels, lower-cost β-lactams like cephalexin were still frequently prescribed, resulting in slightly lower average drug costs in the penicillin-allergic group. These findings support the need for improved allergy documentation, de-labeling strategies, and adherence to evidence-based guidelines to optimize both care quality and cost-effectiveness."
