- "Neuropathic itch may respond to topical anesthetics: menthol, pramoxine, capsaicin, or compounded ketamine-amitriptyline-lidocaine formulations, as well as systemic gabapentinoids and antidepressants. New drugs targeting the mast cell such as BTK inhibitors, c-KIT inhibitors, and MRGPRX2 antagonists have robust anti-pruritic effects in mast cell-mediated diseases. Supportive measures, including psychosocial interventions, remain integral to long-term management."

Innovative Hydrocortisone Acetate and Pramoxine Hydrochloride Topical Cream Formulations for Anorectal Conditions: Enhanced In Vitro Release Profile.

- "This medication combination (hydrocortisone and pramoxine) is the first generic drug product utilized to alleviate minor pain, itching, swelling, and discomfort associated with anorectal conditions such as hemorrhoids. Quantitative content of the release tests of the active ingredients in the cream, assay tests, antimicrobial preservative efficacy, and stability tests were carried out by high-sensitivity liquid chromatography. In conclusion, the cream formulations developed in this study have the potential to offer more effective treatment compared to reference products in terms of both in vitro release rates, and their reliability and validity were confirmed through validation studies."

Self-Assembling Hemostatic Peptide Gel Used to Successfully Treat Rectal Ulcer in Radiation-Associated Vascular Ectasia: A Case Report.

- "Conservative therapy with topical pramoxine was ineffective, so she underwent colonoscopy, which revealed a non-bleeding rectal ulcer in the setting of bleeding radiation-associated vascular ectasia...CONCLUSIONS The mucosal repair properties of the self-assembling hemostatic peptide gel, an endoscopic product initially designed for hemostasis, are highlighted by this case in which a non-hemorrhagic ulcer was successfully treated. Furthermore, this is a novel use of the self-assembling hemostatic peptide gel in the setting of radiation-associated vascular ectasia and may represent a useful addition to the endoscopist's toolkit for managing a challenging condition."

Streptococcal Proctitis in a Healthy Adult Female Following Scarlet Fever

- "Case Description/ A 36 year old female with no significant past medical history presented to the ED for dysphagia, diffuse rash, and malaise, found to have Scarlet Fever and Streptococcal pharyngitis treated with a 10 day course of amoxicillin...She was prescribed empiric perianal Streptococcus treatment with mupirocin and neomycin/polymyxin/pramoxine...It is important to consider other etiologies on the differential diagnosis including irritant and contact dermatitis, inflammatory bowel disease, and sexual abuse. Streptococcal proctitis typically resolves within 14-21 days, although there exists a theoretical complication of rheumatic fever, and there have been reports of post-Streptococcal nephritis following perianal Streptococcal infection."

- "First-line treatment for inflammatory chronic pruritus includes topical anti-inflammatory therapies such as hydrocortisone (2.5%), triamcinolone (0.1%), or tacrolimus ointment...In these patients, referral to dermatology and systemic oral or injectable treatments such as dupilumab or methotrexate may be considered...In these patients, neuropathic topical treatments such as menthol, pramoxine, or lidocaine can be used either alone or in combination with immunomodulatory agents such as topical steroids. Other effective therapies for neuropathic pruritus include gabapentin, antidepressants such as sertraline or doxepin, or opioid receptor agonist/antagonists such as naltrexone or butorphanol. PREVALENCE OF ILEAL, POUCH, AND RECTAL CUFF INFLAMMATION 12 MONTHS AFTER ILEOSTOMY CLOSURE

- "Key variables examined included endoscopic evidence of ulcers or inflammation (including mucosal friability and biopsy confirmed inflammation) in the pouch rectal cuff and pre-pouch ileum clinical symptoms of pouchitis/cuffitis one year after surgery and treatment with proctofoam or a suppository... 97 patients (61 male patients and 36 female patients) underwent a pouchoscopy 1 months after their final surgery. Patients' age ranged from 14 to 75 years and average age was 38 years. 56 patients (58%) including female and 34 male patients had ulcers/inflammation identified at least one site."

Itch Intensity and Therapeutic Management of Pruritus in the Real-World Setting

- "Percentages of patients with prescriptions for any of the following treatments were tabulated by itch intensity: topical treatments (corticosteroids [TCS], calcineurin inhibitors [TCI], capsaicin, menthol, pramoxine/lidocaine/prilocaine, doxepin), systemic treatments (non-sedative antihistamines [NSAH], sedative antihistamines [SAH], opioid receptor antagonists [ORA], and selective serotonin reuptake inhibitors [SSRI], doxepin). Results provide insights into real-world therapeutic management of pruritus as it relates to itch intensity. Symptom severity was strongly associated with greater prescriptions of most therapy classes. Future analyses would be helpful to understand the landscape of pruritus therapeutic management as it relates to itch intensity and symptom reduction."

[VIRTUAL] Safety and Efficacy of an Avena Sativa (Oat) Skin Care Regimen for Therapy-Related Xerosis and Pruritus in Adult Oncology Patients Undergoing Systemic Cancer Treatments

- "A five-week (+/- 1 week) clinical study was performed to determine the safety and efficacy of an Avena Sativa (Oat) skin care regimen (Body Wash, Cream and 0.5% Pramoxine Balm) in improving skin dryness and itch of adult patients undergoing systemic oncology treatments...Improvements in skin moisture, itch and comfort were also perceived by patients at 2 weeks. The Avena Sativa (Oat) daily skin care regimen not only demonstrated significant dry skin and itch reduction, but also improved patient QOL, suggesting this regimen is effective for management of mild to moderate dermatologic side effects faced by adults undergoing systemic oncology treatments."

[VIRTUAL] Allergic Contact Dermatitis to Sodium Metabisulfite in Lidocaine with Epinephrine

- "His usual medications included esomeprazole, celecoxib, pregabalin, acetaminophen and hydroxyzine.Physical examination revealed numerous erythematous papules and vesicles on poorly-circumscribed, eczematous plaques...Results of his patch test read at 96-hours: sodium metabisulfite (+++), bacitracin (+++), polymyxin-B (+++), tixocortol-21-pivalate (+++), pramoxine (++) and linalool (+).ConclusionsThe acute eruption within one day postoperatively, along with clustering of lesions around lidocaine infiltration sites and positive patch test results, suggest that the patient had developed a systemic contact dermatitis to sodium metabisulfite, present in the lidocaine with epinephrine administered perioperatively...He was started on topical clobetasol ointment and recovered in a few weeks.Learning Objective1.True allergy (Type I hypersensitivity) to local anesthetic is rare.2.Sodium metabisulfite is an antioxidant present in local anesthetic solutions with vasoconstrictors,..."
