FATAL TRACHEOBRONCHIAL ASPERGILLOSIS AFTER H1N1 INFLUENZA INFECTION

- "The patient was initiated on peramivir and broad-spectrum antibiotics.Despite gradual improvement in renal function, the patient's respiratory status deteriorated, necessitating high-flow nasal cannula oxygen and intermittent noninvasive ventilation. Subsequent sputum cultures grew Aspergillus fumigatus, prompting the initiation of voriconazole therapy... This case highlights the potential for fatal TBA following H1N1 influenza infection, even in patients without classic immunosuppressive risk factors. Clinicians should maintain a high index of suspicion for invasive aspergillosis in critically ill influenza patients, particularly those with underlying pulmonary conditions or a history of substance use. Early recognition and prompt antifungal therapy are crucial to improving outcomes in this population."

Omadacycline in the treatment of severe Q fever pneumonia during an influenza epidemic: a case report with literature review.

- "Following antiviral treatment with Maraviroc and antibiotic therapy with omadacycline tosilate (a novel tetracycline-class drug) for Coxiella burnetii infection, the patient's clinical symptoms improved, biochemical markers normalized, and pulmonary imaging showed resolution. This case highlights the potential of tNGS to improve the detection rate of mixed infections in cases of severe pneumonia of unknown etiology. The novel tetracycline drug, such as omadacycline, has demonstrated efficacy against Q fever rickettsial pneumonia, offering a new perspective for clinical diagnosis and treatment."

Syphilis can present with non-specific symptoms over a varied time course and is often known as a "Great Imitator" of disease [1]. This case shows that syphilis testing should be considered in patients who present with a prolonged course of upper head/neck symptoms and in those where malignancy is a concern."

[VIRTUAL] A FUNGUS AMONG US: A RARE CASE OF BLASTOMYCOSIS AND INFLUENZA A H1N1 CO-INFECTION

- "He required non-invasive positive pressure ventilation and methimazole, propranolol, high-dose steroids and diuresis for suspected thyroid storm and ceftriaxone and doxycycline for community-acquired pneumonia...Vancomycin, piperacillin/tazobactam, levofloxacin and fluconazole were started...Oseltamivir was started... To our knowledge, this is the first case of blastomycosis and influenza co-infection reported and it indicates a comparable risk of high mortality. Even without superinfection, there are implications for early therapy of acute blastomycosis. Given that it can transition to chronic pneumonia and rarely, ARDS, with a 50%–89% case-fatality rate, it is important to maintain a high index of suspicion for blastomycosis in a patient from an endemic area who has respiratory symptoms."

This case sheds light on the utility of renal replacement therapy in the management of critically ill patients, even those without pre-existing kidney dysfunction or end-organ damage. Further studies are needed to quantify the extent to which renal replacement may benefit these patients."
