Insurance Coverage of Rosacea Topicals: A Cross-Sectional Review

- " Azelaic acid gel, oxymetazoline hydrochloride cream, ivermectin cream, sulfacetamide sodium–sulfur cleanser, metronidazole gel, minocycline foam, brimonidine gel, and Epsolay benzoyl peroxide (BPO) cream were reviewed across UnitedHealth Group, Elevance (Anthem), CVS Health (Aetna), Cigna, Health Care Service Corporation Blue Cross Blue Shield (BCBS), Kaiser Permanente, BCBS Florida, BCBS Michigan, Centene, and Blue Shield of California. Only two rosacea topicals were consistently covered by major commercial insurers, and many plans imposed quantity limits and step therapy barriers. Interestingly, metronidazole, identified as less effective than the uncovered topicals, was the most covered agent. [5] This disconnect between evidence-based rosacea treatments and insurer restrictions underscores the need for reform and improved access."

Seven antiparasitic drugs, albendazole (ABZ), miltefosine (MLT), ivermectin (IVM), tribendimidine (TBD), praziquantel (PZQ), artesunate (ART), and mefloquine (MEQ), were evaluated for in vitro activity against T. spiralis muscle larvae...In vivo, TBD treatment significantly reduced larval burdens in diaphragm and gastrocnemius muscles and was associated with reduced collagen capsule thickness, inflammation, and fibrosis compared with ABZ-treated controls. This study validated an assay for quantitative evaluation of T. spiralis muscle larvae and demonstrates robust in vitro and in vivo activity of TBD against this stage."

From signal to strategy: a disproportionality analysis of dupilumab-associated rosacea in FAERS with a summary of reported clinical cases.

- "Most patients improved with rosacea-directed therapy (topical ivermectin or metronidazole; anti-inflammatory-dose doxycycline). However, dechallenge or rechallenge patterns and the need for dose-interval extension, temporary interruption, or switching biologic (e.g., lebrikizumab, upadacitinib) in a subset support a drug-related pattern at the reporting level. DAR represents a distinct clinical entity from dupilumab-associated head and neck dermatitis, which is eczematous and typically responds to antifungals or calcineurin inhibitors. While disproportionality signals indicate association rather than incidence or causality and are subject to reporting bias, clinicians should be aware of this potential adverse event to ensure appropriate management."

- "Allergic contact blepharitis to brimonidine eye drops is recognized and, more recently, case reports have described patients developing ACD to topical brimonidine gel [Bangsgaard N, Fischer LAN, Zachariae C. Sensitization to and allergic contact dermatitis caused by Mirvaso® (brimonidine tartrate) for treatment of rosacea - 2 cases...He had been using topical Elocon cream daily (mometasone furoate 0.1%) and was diagnosed with steroid-induced rosacea. He was prescribed oral doxycycline, ivermectin cream and brimonidine gel...We would like to raise awareness that in addition to rebound erythema, topical brimonidine gel is also a cause of severe facial ACD. Patch testing to the patient's own medicaments should be performed in suspected cases."

BI21 Intermittent and progressive pustular psoriasis during chemotherapy cycles in a toddler.

- "Just before his next chemotherapy cycle (cisplatin and etoposide), the rash recurred as a pruritic pustular eruption across the frontal and retroauricular scalp; skin biopsy suggested nonspecific subacute eczematous dermatitis...Topical ivermectin and ketoconazole did not help...A second biopsy was taken due to clinical suspicion of psoriasis, and 10 days of oral prednisolone was given to try to clear his skin for autologous stem cell transplantation (HSCT)...He was treated with topical calcipotriol and mometasone 0.1%, with consideration of acitretin should he not improve. Fortunately, the rash settled with topical therapy and time. After completing proton radiotherapy prior to tumour resection, he had six cycles of maintenance immunotherapy (dinutuximab) with no recurrence of the rash, despite monthly GCSF, and good cell counts, and has now completed his oncological therapy."

- "Despite topical treatments and ciclosporin, his eczema persisted, leading to the initiation of dupilumab...This is a case of hypereosinophilic syndrome with colitis secondary to dupilumab in a patient with positive Strongyloides serology. This case demonstrates the diagnostic difficulty in Strongyloides-positive patients, given that both hypereosinophilic syndrome and Strongyloides hyperinfection can present with colitis, and there is a theoretical risk of disseminated Strongyloides and hyperinfection in patients immunosuppressed with dupilumab."
