CD21 Things can only get redder! Two cases of allergic contact dermatitis to topical brimonidine tartrate gel.
(PubMed, Br J Dermatol)
- "Allergic contact blepharitis to brimonidine eye drops is recognized and, more recently, case reports have described patients developing ACD to topical brimonidine gel [Bangsgaard N, Fischer LAN, Zachariae C. Sensitization to and allergic contact dermatitis caused by Mirvaso® (brimonidine tartrate) for treatment of rosacea - 2 cases...He had been using topical Elocon cream daily (mometasone furoate 0.1%) and was diagnosed with steroid-induced rosacea. He was prescribed oral doxycycline, ivermectin cream and brimonidine gel...We would like to raise awareness that in addition to rebound erythema, topical brimonidine gel is also a cause of severe facial ACD. Patch testing to the patient's own medicaments should be performed in suspected cases."

PA01 Treating scabies with oral ivermectin in infants: series of 12 cases.
(PubMed, Br J Dermatol)
- "Early treatment with oral ivermectin should be emphasized, along with reiteration of the importance of treating contacts to reduce chances of recurrence. Additionally, existing data suggest that oral ivermectin in children weighing < 15 kg is safe, which is also supported by our study."

BI21 Intermittent and progressive pustular psoriasis during chemotherapy cycles in a toddler.
(PubMed, Br J Dermatol)
- "Just before his next chemotherapy cycle (cisplatin and etoposide), the rash recurred as a pruritic pustular eruption across the frontal and retroauricular scalp; skin biopsy suggested nonspecific subacute eczematous dermatitis...Topical ivermectin and ketoconazole did not help...A second biopsy was taken due to clinical suspicion of psoriasis, and 10 days of oral prednisolone was given to try to clear his skin for autologous stem cell transplantation (HSCT)...He was treated with topical calcipotriol and mometasone 0.1%, with consideration of acitretin should he not improve. Fortunately, the rash settled with topical therapy and time. After completing proton radiotherapy prior to tumour resection, he had six cycles of maintenance immunotherapy (dinutuximab) with no recurrence of the rash, despite monthly GCSF, and good cell counts, and has now completed his oncological therapy."

BI36 A case study of hypereosinophilic syndrome in a patient with atopic dermatitis on dupilumab therapy and Strongyloides infection.
(PubMed, Br J Dermatol)
- "Despite topical treatments and ciclosporin, his eczema persisted, leading to the initiation of dupilumab...This is a case of hypereosinophilic syndrome with colitis secondary to dupilumab in a patient with positive Strongyloides serology. This case demonstrates the diagnostic difficulty in Strongyloides-positive patients, given that both hypereosinophilic syndrome and Strongyloides hyperinfection can present with colitis, and there is a theoretical risk of disseminated Strongyloides and hyperinfection in patients immunosuppressed with dupilumab."

Effectiveness of Oral Ivermectin Treatment in Cases Unresponsive to Classical Demodex Blepharitis Treatment.
(PubMed, Turkiye Parazitol Derg)
- "OSDI scoring mean ± SD values before classical Demodex blepharitis treatment and after oral ivermectin treatment were 77.47±5.74 and 6.69±4.71, respectively. In the treatment of Demodex related blepharitis, the use of oral ivermectin is an effective treatment option in cases that do not respond to classical treatment."

Effect of Moringa oleifera and ivermectin nanoparticles on the immunopathological response during experimental trichinosis in mice.
(PubMed, Gut Pathog)
- "Treatment with nanoparticle formulations of IVM and MOL modulated macrophage-related immune responses by reducing the pro-inflammatory markers iNOS, TNF-α and NF-κB, while increasing the relative gene expression of the anti-inflammatory cytokine IL-10. Combination therapy exhibited superior efficacy in decreasing parasite burden and mitigating intestinal pathology compared to monotherapy."

PF4 Autoantibody Complexes Cause Activation of Integrins αIIbβ3 and αvβ3 and Possible Subsequent Thrombosis and Autoimmune Diseases.
(PubMed, Int J Mol Sci)
- "When plasma PF4 tetramer is increased, active PF4 monomer is generated by autoanti-PF4 and plays a role in disease pathogenesis. Notably, anti-inflammatory cytokine neuregulin-1 and anti-inflammatory ivermectin bind to site 2 and suppress integrin activation induced by RTO/PF4 complex, suggesting that neuregulin-1 and ivermectin are potentially useful to suppress PF4/anti-PF4-mediated inflammatory signals."

Our study underscores the crucial role of IMN in amplifying the NLRP3 inflammasome cascade in DCs during ICD, presenting a promising strategy to enhance the efficacy of HCC chemoimmunotherapy."

Targeted Management of Severe Myocarditis and Myositis in Drug-Induced Hypersensitivity Syndrome Using Precision Medicine
(AHA 2025)
- "He was ultimately diagnosed with DiHS/DRESS (attributable to ibuprofen use), started on dexamethasone, and discharged on prednisone...The patient was started on solumedrol, cyclosporine, and IVIG...To avoid further loss of functionality and potentially life-threatening consequences, the patient was started on tofacitinib with, surprisingly, a notable improvement in clinical symptoms.DiscussionCardiac involvement and skeletal muscle myositis are under-recognized manifestations of DiHS/DRESS, with immune-mediated necrotizing myositis/myocarditis representing the most severe form. Due to the high morbidity and mortality associated with these complications, patients should be aggressively treated with high-dose systemic corticosteroids to avoid fibrosis of the myocardium and irreversible damage. The use of precision medicine can be critical, particularly in treatment-resistant patients, to improving clinical outcomes and providing expedited targeted medical therapy."

Strongyloidiasis: A Mimicker of Recurrent Colitis and Failure to Thrive
(ACG 2025)
- "She received two days course of oral ivermectin with symptomatic improvement. This case illustrates the diagnostic ambiguity as strongyloidiasis mimics recurrent colitis, asthma exacerbations, pulmonary embolism.Diagnosing strongyloidiasis requires a high index of suspicion due to nonspecific presentations, intermittent larvae shedding in stool.Initial testing may not reveal strongyloidiasis hence recurrent presentations and delayed diagnosis...The overlapping features of parasitic infection, inflammatory colitis, and malignancy require careful consideration. Steroid can trigger severe strongyloidiasis necessitating repeat testing before use."

(ACG 2025)
- "He was treated with praziquantel for two separate 3 day courses over 3 months as well as ivermectin due to concern for concomitant Strongyloides infection...Within the literature one study has shown that the 5-year survival rate amongst patients with CRC cancer with concomitant schistosomiasis is around 45.6% compared to 50.9% of non-infected patients with further studies supporting this finding. These findings highlight the importance of recognizing patients at increased risk and underscore the need for further research evaluating association between schistosomiasis and CRC cancer.Figure: A.) Efficacy of topical Ivermectin in controlling human Demodex infestation: Evidence from systematic review and meta-analysis.
(PubMed, Parasite Epidemiol Control)

However, the potential for mite repopulation after treatment underscores the importance of ongoing monitoring. Study heterogeneity and the limited number of included trials warrant cautious interpretation of the findings."
